User Experience Regarding Digital Primary Health Care in Santarém, Amazon: Evaluation of Patient Satisfaction and Doctor's Feedback.

Background: With the arrival of the pandemic, telemedicine has been widely used to provide medical care and can be used to assist patients in regions far from urban centers that are difficult to access, such as riverside communities in the Brazilian Amazon region. A telemedicine project connecting São Paulo, a mega-metropolis, to Paysandú, a riverside district in the Amazon, was built to serve the local population where access to the nearest medical care is 6 hours away by speedboat.

Objective: This study aims to assess the feedback from patients and doctors regarding the use of telemedicine in outpatient care at Paysandú, a riverside district in the Amazon.

Intraepithelial lymphocyte immunophenotype: a useful tool in the diagnosis of celiac disease.

According to new ESPGHAN guidelines, gluten challenge is considered necessary when there is doubt about the initial diagnosis of celiac disease (CD). The main aim of this study was to quantify intraepithelial lymphocyte (IEL) immunophenotype on celiac patients on gluten-containing diet (GCD) compared to those on gluten-free diet (GFD). Another aim was to evaluate the clinical utility of IELs in the CD diagnosis, especially in selected patients on GFD where diagnostic uncertainty remains.

Neutral detergent-soluble fiber improves gut barrier function in twenty-five-day-old weaned rabbits.

The effect of neutral detergent-soluble fiber level on gut barrier function and intestinal microbiota was examined in weaned rabbits. A control diet (AH) containing 103 g of neutral detergent-soluble fiber/ kg of DM included alfalfa hay as main source of fiber. Another diet (B-AP) was formulated by replacing half of the alfalfa hay with a mixture of beet and apple pulp resulting in 131 g of soluble fiber/kg of DM.

[Antineutrophil cytoplasmic antibodies in nonvasculitis diseases: clinical correlates and target antigens].

The value of antineutrophil cytoplasmic antibodies (ANCA) in the diagnosis of several types of idiopathic vasculitis has been well-documented: In these diseases the ANCA show two classical immunofluorescence patterns, C-ANCA and P-ANCA, which have antigen specificity for the myeloperoxidase and proteinase 3, respectively. However, the appearance of ANCA in very different diseases other than the mentioned vasculitis, has been documented in recent years. In these diseases, the ANCA generally have atypical immunofluorescence patterns and are directed against neutrophil antigens that are different from the previous two, their clinical value still being under debate.
