Pharmacokinetics of propranolol (P), 4-hydroxy-propranolol sulfate (4HOP-Sulf), and glucoronides of pharmacologically active S-enantiomer P (S-PG) and ballast R-enantiomer of P (R-PG) in the blood serum of 21 patients with chronic ischemic heart disease and/or arterial hypertension has been studied at a single and course oral P administration. The values od AUC and T1/2 for potentially active 4HOP-Sulf were significantly higher than those for unchanged P at a single and course administration. The values od AUC and T1/2 for for S-PG were approximately three times higher than those for P-PG after both a single and course administration.

A multicenter, controlled, open, randomized trial of antihypertensive and life quality (LQ) effects of monotherapy with captopril vs propranolol, hydrochlorothiazide and nifedipine included 345 males aged 35-60 years with mild and moderate hypertension. The results of 8-month treatment provided the conclusions: the drugs were much alike by antihypertensive efficacy, LQ parameters proved more beneficial in the group on captopril than those on nifedipine and propranolol, hydrochlorothiazide failed to improve life quality, captopril is an effective and safe antihypertensive drug superior to the drugs of 3 other antihypertensive drug groups by positively affecting LQ.

The new original Russian drug proxodolol having alpha- and beta-adrenoblocking activities in a ratio of 1:100 was examined at Stages I and II of clinical trials. The drug was given to 29 male patients aged 30-63 years who suffered from Stages I and II persistent hypertensive disease with a baseline diastolic blood pressure of >95 mm Hg. Proxodolol as 10- and 40-mg tablets was demonstrated to be an effective antihypertensive agent, its antihypertensive effects being revealed in acute and chronic uses.

The cardioselective beta-adrenoblocker acebutolol used as a course therapy for 12 weeks was found to be a highly beneficial antihypertensive agent. The antihypertensive effect of the agent given in doses of 400-800 mg/day was as pronounced and prolonged as that of propranolol, 80-160 mg/day, though there is a tendency for acebutolol to show its complete or partial antihypertensive effect rather at the end of monotherapy than propranolol. At the same time the bradycardiac effect was more pronounced in propranolol therapy.

Nitrendipine (bypress) manufactured by Bayer Company in the form of 10 and 20 mg tablets and administered in a dose of 20-40 mg/day turned out an effective antihypertensive remedy in 65% of patients with arterial hypertension largely running a mild course. The effect of nitrendipine administered in a single daily dose of 20 mg did not differ from that attained with the same dose given in two intakes. The raising of the daily dose of nitrendipine from 20 to 40 mg was associated with augmentation of the hypotensive effect in 43.

The paper presents the results of the study into the efficacy of single routine doses of essential antihypertensive agents (nifedipine, propranolol, and hydrochlorothiazide) versus its course treatment at rest and during isometric and dynamic exercise tests. The efficacy was assessed by using the Dixon criterion. The routine drug dose tests performed at rest and during isometric and dynamic exercise were shown to have a varying informative value in the prediction of long-term treatment with the above drugs.

The paper is concerned with some data on the time course of indices of carbohydrate metabolism and lipid spectrum in patients with type II diabetes mellitus with concomitant CHD or arterial hypertension during therapy with mean doses of propranolol. The course treatment with propranolol resulted in the deterioration of carbohydrate metabolic indices and unfavorable shifts in the lipid spectrum of some patients. Patients on insulin therapy might demonstrate a significant decrease in the blood sugar level after physical exercise.

Analysis of the incidence and diagnostic importance of the congenital central nervous system (CNS) defects in 334 children with multiple congenital developmental defects (MCDD) dying under 1 year allowed the author to distinguish 4 groups of defects according to their informative value for establishing the nosological diagnosis: defects of a high, moderate, small informative value and noninformative ones. Special microscopic study of the CNS performed in 116 stillborns and children of the first year of life with various MCDD showed the disturbance of main morphogenetic processes (mitotic cell activity, cell migration, differentiation and maturation) as a basis of the observed pathological conditions. General rules of the CNS developmental defects in chromosomal and non-chromosomal syndromes may point to the common routes of the genetic material realization.

The resting ECGs were analyzed in males aged 20-60 years with various tolerance to carbohydrates. There were statistically significant differences between the values characterizing the functions of automatism and conductivity on the resting ECG in patients with diabetes mellitus without any clinical symptoms of coronary heart disease (CHD), arterial hypertension, cardiac failure and other chronic diseases and in healthy males with the normal tolerance to carbohydrates. in patients with borderline impairments of carbohydrate tolerance (CT), these parameters occupied an intermediate position between the corresponding values in patients with diabetes mellitus and in subjects with the normal tolerance to carbohydrates.

The authors present the data on the prevalence of coronary heart disease (CHD) and its risk factors, on the incidence of new cases of CHD and the associated mortality in the unorganized population of 50-59-year-old men of Moscow as well as findings on the natural variability of risk factors obtained over six years in a follow-up study. The relationship between the CHD incidence on the one hand and the baseline arterial pressure levels, excessive body weight and smoking habit on the other is evaluated. Arterial hypertension is shown to be the most prognostic risk factor of CHD in the unifactorial analysis.

Data on pathological manifestations of hydranencephaly (3 cases) consisting of complete or nearly complete absence of cerebral hemispheres with the intact stem, cerebellum, spinal cord, meninges and skull bones are presented. A number of structural disorders were found in the remaining structures of the brain. A hypothesis on the origin of this central nervous system developmental defect is proposed: hydranencephaly is associated with disorders in the development of leptomeninges at the early stages of embryogenesis.
