From July through September 1987, our emergency department registered 17,214 patients, of whom 569 (3%) returned within two days of initial registration. Cases were reviewed to identify factors associated with return visits. Patient-related factors were responsible for a majority of repeat visits (267 cases, 53%).

HIV-1 was isolated by coculturing of patient peripheral blood mononuclear leukocytes (PBMCs) with normal blood donor target PBMCs that had been stimulated with phytohemagglutinin and interleukin-2. The HIV-1 isolation success rate at Walter Reed increased progressively each year from 1986 to 1989.

View Article and Find Full Text PDFTo identify vaccine relevant T cell epitopes on the circumsporozoite (CS) protein of Plasmodium falciparum, the lymphocyte proliferative responses to 10 CS protein derived peptides were studied in 28 adult Kenyans, and correlated with resistance to malaria. Eight peptides, six of which were not overlapping, induced proliferation of lymphocytes from one to five volunteers, suggesting either genetic restriction of response to each of the T epitopes, or dominance of some T sites on the immunizing sporozoites. The 28 volunteers were radically cured of malaria and during the next 126 days 25 of the 28 were reinfected.

An enzyme-linked immunosorbent assay (ELISA) for circulating IgG mouse antibody to Plasmodium falciparum circumsporozoite (CS) protein was modified for use with human sera collected in an area of northern Zambia that was endemic for malaria and from individuals never exposed to malaria. Optimum sensitivity was achieved using Immulon 2 microtitration plates, boiled casein-Tween 20 blocking buffer, and by adding a solution of boiled casein (4 micrograms/ml) to the capture antigen diluent. The results for the detection of anti-CS IgG correlated well with those of sporozoite immunofluorescence antibody assays.

Portions of splenic or subcutaneous saline aspirates from suspected visceral or cutaneous leishmaniasis patients were inoculated into NNN media with an overlay of Schneider's medium or Schneider's medium alone for routine parasitological diagnosis. The remaining portions of the aspirates were used for preparing Giemsa-stained smears and for subcutaneous inoculation into hind foot-pads of Balb/c mice. Saline aspirates obtained from the foot-pads 2-14 d after inoculation were inoculated into Schneider's medium and examined for promastigotes.

An 18-month sandfly survey was conducted at 4 locations in Baringo District, Rift Valley Province, Kenya. 3 collection techniques were used: aspiration, sticky paper trap, and light trap in sites selected because of their proximity to homes of visceral leishmaniasis patients diagnosed and treated within 6 months before the survey. Patients not cured after initial treatment were rerandomized to one of the other treatment schedules. Overall, 21% of subjects had ELISA reactions suggesting the presence of antibody against HTLV-III.
