The natural killer (NK) activity of peripheral blood lymphocytes of gastric cancer patients in all pathological stages from I to IV was less than that of healthy persons or patients with benign diseases. However, there was no significant difference in the antibody-dependent cellular cytotoxicity (ADCC) of cancer patients and healthy individuals, and the correlation between NK and ADCC activities was somewhat lower in cancer patients than in healthy persons. That blood transfusions aid kidney graft survival is well known. Our data show that blood transfusions, except for the red blood cell component, promote growth of transplanted tumors in mice. We studied the effects of a splenectomy in combination with immunotherapy on the survival of patients who had undergone a total gastrectomy. It was found that a splenectomy was not effective against advanced gastric cancer at stage III, and that the spleen should be retained for immunotherapy. Splenectomy for gastric cancer at terminal stage IV, particularly in combination with immunotherapy, produced not only augmentation of cellular immunity, but also increased survival.

To clarify the clinical efficacy of SPG, a beta-1, 3 glucan extracted from cultured Schizophyllum commune Fries, a randomized comparative study was performed in combination with mitomycin C+5-fluorouracil (MF protocol) or tegafur (F protocol). A total of 514 cases with inoperable or recurrent gastric cancer were randomly allocated to either the SPG group or the control group, 367 of which were finally assessed for clinical efficacy. Immunotherapy with levamisole (LMS) and combined splenectomy were performed in the cases of stages III and IV gastric cancer of the upper and middle stomach and associated with total gastrectomy. The cell-mediated immunity and the survival rates for both groups were compared with controls. The influences of surgical stress on the local graft-versus-host reaction (GvHR) in F1 mice were studied. Skin incision to the F1 mice inflicted to 1 day prior to injection of parental spleen cells produced impairment of popliteal lymph node enlargement, however, this effect was not observed when GvHR was induced on the day 3 and 5 after operation. Spleen cells from hind limb amputated F1 mice exhibited suppressor activity on local GvH reaction when injected into naive syngenic F1 mice together.

The effect of NK-421(Bestatin) and Ge-132 (an organic germanium compound) on the ADCC and natural killing (NK) activities of the spleen cells of MH-134 tumor-bearing mice were studied. In the tumor-bearing mice, the ADCC activity was enhanced, and NK activity was reduced in accordance with the progress of the tumor. By oral administration of Bestatin at doses of 5, 10 and 50 mg/kg, ADCC activity was potentiated, and at a dose of 10 mg/kg, NK activity was significantly increased.

This study was conducted in patients with stage III gastric cancer who were admitted and gastrectomized in our Department of Surgery during July 1971 to March 1980. The control group consisted of 32 patients who were treated with mitomycin C and FT -207 during the first half of the period and the levamisole (LMS) group of 32 patients who were treated with the same regimen plus LMS during the latter half of the period. BCG-CWS was administrated into lymphatic vessels in the treatment of advanced malignant melanoma metastasized to the lymph node of the posterior peritoneum. Side effects consisted of fever, lymphangitis, a decrease in urinary output and a slight rise in serum GOT, there being no serious side effects. Clinically, tumor mass in the abdomen cleared but metastases to the other organs increased, and the patient died of general dispersion of the disease.

A female patient who died of apparent postirradiation sarcoma at the age of 39 was reported.
