Diffuse malignant pleural mesothelioma is a rare primary tumor of the pleura with three principal histological types, epithelial; mesenchymal and mixed epithelial and mesenchymal. We report here a case of a mesenchymal mesothelioma with foci of osteosarcomatous degeneration revealed by dense calcifications associated with the pleural effusion on the computed tomography (CT) of the thorax. The bone scan revealed extraosseous uptake corresponding to the left pleura.

Bone lesions are fairly uncommon in sarcoidosis (5 to 10% of cases). We report the case of a 40-year-old man in whom sarcoidosis of the lungs and bones was revealed by excruciating buttock and sacral pain. Computed tomography showed multiple punched-out defects in the left iliac bone.

Inoperable non- microcellular primary bronchial carcinomas have been reputed up to now to be chemo-resistant. The introduction of Cis-platinum into a polychemotherapy protocol leads to revision of this concept. The authors report the preliminary results of a polychemotherapy protocol (including Cis-platinum, Vindesine, CCNU, Cyclophosphamide) associated, in cases of non- metastasized carcinomas, with radiotherapy to the tumour itself, the mediastinum and the supraclavicular fossae.

Based on the technique described by Gabler et al. several years ago, stabilization of the mediastinum after penumonectomy is obtained by introducing a blown up latex balloon into the operated side of the thorax. The balloon is introduced immediately following surgery after being distended by means of Kuss's apparatus, and is removed, after being emptied in the same way, five to seven days later.

Beyond the usual respiratory functional tests, ergometry represents a complementary test, yielding much information on the functioning of the lung-heart system. Simultaneous recording, studying and comparing different respiratory and haemodynamic functional parameters during effort, provides an objective appreciation of the cardio-respiratory energetic reserve. Such a study is necessary to estimate a functional handicap and to understand the pathophysiological alterations of the cardio-respiratory system.
