Article Synopsis
  • Poor oral health during stroke recovery can lead to malnutrition and hinder a patient's ability to perform daily activities, ultimately affecting their chances of going home.
  • This study analyzed 137 stroke patients based on their Oral Assessment Guide scores, comparing those with normal oral health to those with impaired oral health.
  • Results showed that a higher percentage of patients with normal oral health achieved independence in daily activities, emphasizing the importance of addressing oral issues early in recovery.
  • However, there was no significant difference in the rate of home discharges between the two groups, indicating that while oral status impacts daily functioning, it may not directly influence home discharge outcomes.

The area under the concentration-time curve (AUC)/minimum inhibitory concentration (MIC) - guided approach is recommended for vancomycin therapeutic drug monitoring in severe methicillin-resistant (MRSA) infection. However, evidence regarding the efficacy of vancomycin AUC-guided strategies for the treatment of systemic infections is limited. This case report describes the successful treatment of MRSA meningitis, with vancomycin using a higher AUC/MIC target.

  • Enteral nutrition is crucial for stroke patients who struggle with eating, but traditional feeding methods are time-consuming and can disrupt care.
  • The Rapid EN trial aims to compare the safety of quickly administering enteral nutrition (100 mL in 5 minutes) versus the conventional method (100 mL in 30 minutes) in acute stroke patients.
  • The study will track complications like vomiting and pneumonia within 7 days, assessing if the faster method is equally safe while also looking at overall treatment times and clinical outcomes.

Anamorelin (ANAM) is a novel ghrelin receptor agonist for the treatment of cancer cachexia. In clinical trials of ANAM, glucose metabolism disorders as adverse effects were relatively frequent, however, when and how they occur remains unclear. Moreover, the safety in patients with pancreatic cancer and/or diabetes has not been clarified because most previous studies focused on patients with non-small cell lung cancer and had excluded patients with poorly controlled diabetes.

Background: The clinical benefits of faster recanalization in acute large vessel occlusion are well recognized, but the optimal procedure time remains uncertain. The aim of this study was to identify patient characteristics that necessitate puncture-to-recanalization (P-R) time within 30 min to achieve favorable outcome.

Methods: We evaluated the patients from a prospective, multicenter, observational registry of acute ischemic stroke patients.

Article Synopsis
  • The study investigated the impact of treatment timing (on-duty vs. off-duty hours) on outcomes for patients undergoing endovascular treatment for acute ischemic stroke.
  • It analyzed data from 1,571 patients in a multicenter registry, focusing on the modified Rankin Scale (mRS) at 90 days post-stroke as the primary outcome measure.
  • Results indicated no significant difference in mRS scores between on-duty and off-duty patients, but off-duty patients experienced longer door-to-puncture times, leading to poorer outcomes when that time exceeded 60 minutes, influenced by factors like low NIHSS scores and diabetes.

Background: It has been reported that patients with acute myocardial infarction (AMI) transferred to low-volume primary percutaneous coronary intervention (PCI) hospitals (<115/year) in low population density areas experience higher in-hospital mortality rates. This study compared in-hospital outcomes of patients admitted to high-volume primary PCI hospitals (≥115/year) with those for other regional general hospitals.

Methods and results: Retrospective analysis was conducted on data obtained from 2,453 patients with AMI admitted to hospitals in Iwate Prefecture (2014-2018).

Article Synopsis
  • This study evaluated how effective and safe thrombectomy is for acute ischaemic stroke patients who had some pre-stroke disability, using data from 2313 patients.
  • Out of the 2136 patients analyzed, 33.3% with pre-stroke disabilities achieved a favorable outcome (mRS score of 0-3) after 90 days, with NIHSS scores being a significant predictor of success.
  • The findings suggest that both intravenous t-PA and thrombectomy are safe and particularly beneficial for patients with lower NIHSS scores experiencing stroke-related disabilities.

Herein, we investigated possible practical issues for the smooth implementation of the revised Japanese Guidelines for Non-clinical Studies of Vaccines for the Prevention of Infectious Diseases, which were raised in response to public comments on the proposed guideline revision and a gap analysis of the World Health Organization and European Medicines Agency guidelines. We identified main issues such as the non-clinical safety studies of adjuvants and evaluation of local cumulative tolerance in toxicity studies. The revised Japanese Pharmaceuticals and Medical Devices Agency (PMDA)/Ministry of Health, Labour and Welfare (MHLW) guidelines require non-clinical safety studies for vaccines containing new adjuvants, but additional safety pharmacology studies or safety studies in two animal species may be required if non-clinical safety studies raise any concerns (i.

Escherichia coli has multiple pathways to release nonproductive ribosome complexes stalled at the 3' end of nonstop mRNA: tmRNA (SsrA RNA)-mediated trans-translation and stop codon-independent termination by ArfA/RF2 or ArfB (YaeJ). The arfA mRNA lacks a stop codon and its expression is repressed by trans-translation. Therefore, ArfA is considered to complement the ribosome rescue activity of trans-translation, but the physiological situations in which ArfA is expressed have not been elucidated.
