Background: The significance of pulmonary artery (PA) diameter in patients with non-small cell lung cancer (NSCLC) and chronic obstructive pulmonary disease (COPD) who undergo pulmonary resection has not been elucidated.

Methods: Data of 357 patients with NSCLC and COPD who underwent pulmonary resection were retrospectively reviewed. The main PA diameter, determined by preoperative computed tomography, relative to the body surface area (PBR), was used as an index of PA dilatation, and patients were divided into 2 groups using median values.

Although direct oral anticoagulants (DOAC) have become widely used, little is known about the efficacy of DOAC for isolated distal deep vein thrombosis (DVT). In-hospitalized orthopedic patients with isolated distal DVT who were diagnosed from 2016 to 2018 were enrolled and were followed for 1 year. Embolic events included symptomatic pulmonary embolism (PE) and DVT extension above the knee.

Background: The prognosis of patients with synchronous oligometastatic non-small cell lung cancer (NSCLC) has been improving owing to advancements in imaging techniques and new treatment approaches such as tyrosine kinase inhibitors. This study aimed to investigate the long-term outcomes, including the clinical course after recurrence, of patients with synchronous oligometastatic NSCLC with only brain metastases, treated with bifocal treatment.

Methods: We retrospectively analyzed 22 patients with clinical T1-4 and N0-1 NSCLC with synchronous brain metastases who were diagnosed by preoperative PET/CT and brain CT or MRI and underwent pulmonary resection for the primary site and surgery or radiation therapy for brain metastases at our institution from 2005 to 2019.

Key Clinical Message: In addition to the reentrant map, the wave speed map can be helpful in accurately identifying the CTI gap during radiofrequency application for atrial flutter(AFL). However, in complex cases involving extensive scarring and multiple low-velocity local areas, this technique may not be useful.

Abstract: A 73-year-old male patient with a history of pulmonary vein isolation and cavotricuspid isthmus ablation underwent a second catheter ablation owing to recurrent atrial flutter (AFL).

Article Synopsis
  • An 85-year-old woman's severe hyponatraemia (low sodium levels) led to transient loss of consciousness and subsequent cardiac dysfunction, ultimately diagnosed as Takotsubo syndrome, indicating a heart condition triggered by stress.
  • The patient was treated with hypertonic saline to gradually correct her low sodium levels, which required careful monitoring to avoid complications, and her condition improved over 14 days.
  • This case highlights the importance of promptly treating severe hyponatraemia in elderly patients, raises concerns about current treatment protocols for mineralocorticoid-responsive hyponatraemia (MRHE), and suggests a possible link between MRHE and acute cardiomyopathy.

Aims: Left atrial (LA) volume index (LAVI) in chronic heart failure (HF) predicts cardiovascular outcomes. However, the association between LAVI reduction during acute decompensated HF (ADHF) and its prognostic potential is limited. We hypothesized that LA reverse remodelling (LARR) after ADHF therapy would be associated with better clinical outcomes.

Objectives Among patients with advanced non-small cell lung cancer (NSCLC), there are those who do not have a spouse, family members, or friends to support them in their cancer treatment and daily life: the kinless patients. Therefore, we designed an exploratory study of kinlessness and the prognosis of advanced NSCLC. Methods We retrospectively analyzed the prognosis of clinical factors and treatment and kinlessness in patients with advanced NSCLC with wild-type or unknown status for epidermal growth factor receptor/anaplastic lymphoma kinase (EGFR/ALK) and who visited our hospital from November 2018 to February 2023.

Background: Although very late stent thrombosis (VLST) remains an important concern, the underlying etiology and clinical characteristics are not fully elucidated in Japanese patients who undergo intravascular imaging-guided percutaneous coronary intervention (PCI) regularly.

Methods: We identified 50 VLST lesions (bare-metal stent [BMS] [n = 16], first-generation drug-eluting stent [DES] [n = 14] and newer-generation DES [n = 20]) in patients managed in our institutes. The underlying mechanism of VLST was assessed by optical coherence tomography (OCT), and the major etiology of each lesion was determined.

Article Synopsis
  • Drug-coated balloons (DCBs) are effective in treating coronary artery disease (CAD), but their impact on neointimal changes is not well understood.
  • In a study of 12 lesions from 11 patients, it was found that two-thirds showed progression of neoatherosclerosis, while a smaller portion showed regression, with some lesions displaying new calcifications and structural changes.
  • These findings suggest varied neointimal reactions post-DCB treatment, highlighting the need for further research in larger populations to grasp their clinical implications.

A synchronous presentation of pulmonary lesions of Langerhans cell sarcoma (LCS) and Langerhans cell histiocytosis (LCH) is extremely rare. We report the case of a 57-year-old man with 3 pulmonary nodules, 1 in the left lung and 2 in the right lung. In a 2-stage operation, he first underwent a left segmentectomy, and the nodule was diagnosed as LCS.

AbstractThe locomotion strategy of cephalopods is an important factor that influences their ability to exploit various oceanic environments. Particularly, cuttlefish have a unique locomotion strategy; they prefer slow walking (ambling) on the seafloor over swimming. For this locomotion, they use their ventral arms as forelimbs and ambulatory flaps as hindlimbs.

Spinal cord ischemia (SCI) after endovascular abdominal aortic aneurysm repair is a rare but devastating complication. Occlusion of the artery of Adamkiewicz or feeders to the collateral network for spinal cord circulation (such as the subclavian, intercostal, lumbar, and internal iliac arteries) is associated with the onset of SCI. We present a case of monoplegia owing to SCI after elective endovascular abdominal aortic aneurysm repair with coil embolization of the left internal iliac artery in an elderly patient with a history of arteriosclerosis obliterans and aortic dissection, preoperatively occluding multiple intercostal arteries and the right internal iliac artery.

Chronic consumption of excess ethanol is one of the major risk factors for colorectal cancer (CRC), and the pathogenesis of ethanol-related CRC (ER-CRC) involves ethanol-induced oxidative-stress and inflammation in the colon and rectum, as well as gut leakiness. In this study, we hypothesised that oral administration of sesaminol, a sesame lignan, lowers the risk of ER-CRC because we found that it is a strong antioxidant with very low prooxidant activity. This hypothesis was examined using a mouse model, in which 2.

Article Synopsis
  • Type A thymomas are typically benign, spindle/oval-shaped tumors with low risk of metastasis, often detected in early stages.
  • A case is presented of a 62-year-old patient who had a small (12 mm) type A thymoma that surprisingly metastasized to the lung, despite its small size.
  • The findings suggest that even small type A thymomas (under 20 mm) can potentially lead to pulmonary metastasis, challenging the assumption that size correlates directly with metastatic potential.

Objectives: Metastasectomy is often the local treatment for pulmonary metastases arising from osteosarcoma or soft tissue sarcoma. However, there have been few investigations on the outcomes of patients who undergo this procedure. In this study, we identified prognostic factors in patients with pulmonary metastases arising from osteosarcoma and soft tissue sarcoma to determine more appropriate eligibility criteria for metastasectomy.
