Head and neck microvascular reconstruction is a complex but powerful tool following immediate cancer ablation or delayed sequalae after cancer treatment. Virtual surgical planning (VSP), using computer-aided design or computer-aided manufacturing has emerged as a potentially beneficial adjunct to head and neck reconstruction. The aim of this study was to assess the outcomes of VSP technology using the KLS Martin custom-made cutting guides and low-profile plates in head and neck microvascular reconstruction in our centre.

Article Synopsis
  • Anterior mandible defects impact support for essential functions like airway, feeding, and speech, which can result in complications for patients.
  • In a study involving four patients with these deformities, reconstruction was achieved using a fibula osteocutaneous flap for the mandible and a soft tissue flap for the lip and neck, with all flaps successfully surviving.
  • Post-surgery, patients transitioned from tube feeding to oral diets, tracheostomies were removed, drooling ceased, and speech improved significantly, demonstrated by a marked increase in the functional assessment score.

Background:  Microsurgery is essential in modern reconstructive surgery and plastic surgery training. Surgeon's proficiency and experience are crucial for effective microsurgical interventions. Despite anecdotal evidence of varying quality of microsurgery training globally, no empirical studies have investigated this.

Background: In September 2022, a new Ebola outbreak was reported in Uganda, East Africa, and 142 confirmed cases, including 19 Healthcare workers (HCWs) reported. Ebola is not endemic in Somalia, but the country is at a reasonable risk of the virus being introduced due to the direct connection with daily flights from Uganda without border health control and prevention activities. Therefore, evaluating HCWs' Knowledge and attitude is crucial since this is the first time being evaluated in Somalia.

Unlabelled: Opportunistic infections (OIs) remain the leading cause of death among people living with Human immune deficient virus and OIs-related mortality in Africa is estimated at 310 000 cases. Besides, Somalia has scant data about OIs since a high burden of tuberculosis and HIV co-infection has been reported. Hence, up-to-date information is vital for better treatment and interventions and may support national and international HIV strategies and eradication programs.

Unlabelled: Measles is endemic in Somalia; recurrent outbreaks are reported annually. Under-five children are the most affected due to low immunization coverage, vitamin A deficiency, and malnutrition. The study aims to evaluate the demographical, clinical, and complication variations between vaccinated and unvaccinated hospitalized children with measles in the study hospital.

Background: Malignant ovarian germ cell tumors represent small percentage of malignant ovarian neoplasms but they affect significantly young age group.

Aim Of The Study: To investigate the immunohistochemical expression of p16 tumor suppressor protein in malignant ovarian germ cell tumors.

Materials And Methods: Twenty-two malignant ovarian germ cell tumors (five dysgerminoma, eight immature teratoma, and nine yolk sac tumors), twenty mature cystic teratoma tumors and twenty normal ovarian tissue were immunohistochemically stained with p16 monoclonal antibody.

Purpose: Neurotrophic keratopathy (NK) is an uncommon but challenging clinical condition characterized by altered corneal nerves and sensation leading to corneal damage. Corneal neurotization, a surgical technique that aims to "re-innervate" the cornea, has gained increasing popularity in view of the potential to permanently improve or even restore the normal corneal sensation. In this study, we aimed to report the outcomes of two cases of NK that underwent indirect minimally invasive corneal neurotization (MICN) with a sural nerve autograft, and to provide plausible explanations for the observed clinical outcomes.

Introduction: Annular pancreas is an uncommon congenital abnormality characterized by a ring of pancreatic tissue around the duodenum descending part. It is exceptionally rare in adults and is increasingly detected after the investigation of symptoms caused by its consequences, which include recurrent pancreatitis, duodenal stenosis at the site of the annulus, or duodenal or gastric ulcers. In this study, we present a case report of symptomatic annular pancreas in an adult patient.

Background: Electrical injuries of the scalp are a frequent occurrence in developing countries. Burns can be contact or conductive and result in extensive tissue damage. The authors present their experience with treatment of scalp and calvarial electrical injuries and propose a management algorithm.

Article Synopsis
  • - The study analyzed postoperative radiotherapy (PORT) delivery in head and neck cancer patients who underwent free flap reconstruction to assess delays and their impact on treatment outcomes.
  • - Out of 87 patients, 32 received PORT within six weeks of surgery, with delays categorized as surgery-derived, system-derived, or patient-derived; only a small percentage experienced delays due to surgical complications.
  • - The findings suggest that complications from the free flap surgeries do not significantly hinder the timely delivery of PORT in these patients.

Article Synopsis
  • The debate over vasopressor use in microsurgery has been ongoing, with traditional views advocating against their use due to concerns about poor perfusion and complications during free tissue transfer surgery.
  • A retrospective review of 777 cases at a high-volume center showed that 59.1% of patients received vasopressors without an increased flap failure rate, but there was a noted increase in microvascular thrombosis.
  • The study concludes that continuous administration of vasopressors could be safer compared to intermittent bolus doses, supporting the idea that their use in surgery may not be as harmful as previously thought.

The emergence of coronavirus disease 2019 (COVID-19) as a pandemic threatens the entire world resulting in severe consequences for people's health. Pregnant patients with COVID-19 had immune dysregulation that could result in abnormal pregnancy outcomes such as hydatidiform mole (HM), recurrent pregnancy loss, and early-onset preeclampsia. In this article, we tried to summarize the possible association between COVID-19 and the HM's development by reviewing the role of NOD-Like Receptor (NLR) Family Pyrin Domain Containing 7 (NLRP7), cytokines, zinc, and leukocytes in the pathogenesis of HM.

Objective: Merkel cell carcinoma (MCC) is a rare, aggressive skin tumor. Controversies regarding optimal management persist due to inadequate data and knowledge regarding tumor biology. Head and neck MCC increases both oncological and reconstructive challenges, compounded by predominantly elderly patients.

The radial forearm flap is a commonly used method of intra-oral reconstruction, but problems with the donor site are not uncommon. In an attempt to avoid complications, Elliot et al described the use of an ulnar-based flap to obtain direct closure. Where it was not possible to close the proximal defect using V-Y closure, the use of a split skin graft was recommended.
