Article Synopsis
  • T cell pathology in the skin causes an influx of monocytes, but we lack knowledge about how these recruited cells behave over time and affect immune balance in the skin.
  • Research combining a mouse model of acute graft-versus-host disease (aGVHD) and patient samples reveals that disease leads to the differentiation of macrophages specifically in the skin's dermis and results in a dominance of these macrophages, reducing the presence of resting MHCII cells.
  • After the disease resolves, exposing the altered skin to certain substances can cause overactivation of regulatory T cells (Tregs), leading to a loss of immune tolerance and an enduring impact on immune regulation, referred to as an "immunological scar."

A 7-year-old girl presented with a painless neck swelling localised near the left lobe of the thyroid gland, which was initially investigated by fine needle aspiration cytology. This raised a differential diagnosis of medullary thyroid carcinoma and small round blue cell tumour. Only after several additional clinical investigations and a total thyroidectomy was a definitive diagnosis of spindle cell tumour with thymus-like differentiation (SETTLE) reached.

A 70-year-old woman presented with a 10-month history of an irregular mass in the left lateral nape of her neck which had recently increased in size rapidly. Ultrasound-guided core needle biopsy was obtained, and the tumour was diagnosed as a well-differentiated squamous cell carcinoma. Further imaging studies failed to demonstrate additional malignant characteristics.

Selecting the most representative site for biopsy is crucial in establishing a definitive diagnosis of oral epithelial dysplasia. The current process involves clinical examination that can be subjective and prone to sampling errors. The aim of this study was therefore to investigate the use of optical coherence tomography (OCT) for differentiation of normal and dysplastic oral epithelial samples, with a view to developing an objective and reproducible approach for biopsy site selection.
