Mild glucose intolerance (positive urine glucose or mild hyperglycemia) during health checkups in young, lean women may require specialist consultation. We herein report a 31-year-old Japanese woman with mild hyperglycemia detected during a checkup who was diagnosed with overt diabetes in pregnancy and HNF4A-MODY postpartum, without prior follow-up. This case highlights the following: MODY may be present in young, lean women with mild glucose intolerance, and preconception care may improve pregnancy outcomes.

Article Synopsis
  • Immune checkpoint inhibitors (ICIs) have improved cancer treatment outcomes but identifying effective biomarkers for patient selection is essential due to current markers like PD-L1 showing limited predictive accuracy.
  • * This study analyzes blood-based factors, like neutrophil-to-lymphocyte ratio and red cell distribution width (RDW), to predict benefits from ICI therapy using data from a Clinical Data Warehouse (CDW) and electronic medical records (EMR).
  • * Results showed a significant correlation between high RDW and the likelihood of receiving fewer doses of the ICI pembrolizumab, highlighting its potential as a prognostic marker for predicting treatment efficacy.

Background/aim: Physical decline is accompanied with malnutrition in advanced cancer patients, thus nutritional care is often provided with cancer rehabilitation. However, a limited number of studies have focused on which nutritional index serves as an important marker to provide more intensive nutritional support for patients.

Patients And Methods: We retrospectively reviewed advanced cancer patients who received chemotherapy and rehabilitation during hospitalization.

  • Benralizumab, an antibody for eosinophilic asthma, helps reduce flare-ups and reliance on corticosteroids, but its prescribing varies significantly across Japan’s 47 prefectures.
  • A study using national insurance claim data found that higher average income correlates with increased benralizumab prescriptions, while correlations with physician density were weaker.
  • The findings highlight that income is the most significant factor affecting access to advanced asthma treatments in Japan, suggesting that addressing financial barriers is crucial for improving patient care and outcomes.

Analysis of volatile hydrocarbons in blood from fire-related deaths provides useful information such as whether the victim inhaled smoke from the fire before death or whether an accelerant was used in the fire. In this study, we used headspace gas chromatography-mass spectrometry to quantify volatile hydrocarbons in post-mortem heart blood from 121 fire victims. The cases were classified into the following four groups according to the detected volatile hydrocarbons: construction fires without accelerants, kerosene fires, gasoline fires, and a group with no fire-related hydrocarbons detected (other fires).

Background: The second demonstration experiment of supporting elderly people going out with the Choisoko system was conducted. The first study showed that for women, friends, shopping, convenience, and events are factors that have the potential to be effective motivational factors for encouraging these women to go out. On the other hand, these factors did not lead to any behavioral change in men.

Key Clinical Message: This case report describes the clinical course of a juvenile female with FGF23-related hypophosphatemic rickets preceding the onset of SLE. Our study demonstrates the possibility of hypophosphatemic rickets as an early symptom of SLE.

Abstract: Fibroblast growth factor 23 (FGF23)-related hypophosphatemic rickets is observed in both genetic and acquired disorders.

Non-obstructive mesenteric ischemia (NOMI) is caused by reduced blood flow to the intestines without physical occlusion in the mesenteric artery. Previous reports show that drug overdose occasionally induces late-onset NOMI; however, in most cases, the reason for the delayed onset is unclear. Here, we present an autopsy case of late-onset NOMI that was induced by a drug overdose.

  • This study focused on Pseudohypoparathyroidism type 1B (PHP1B) and its various causes linked to methylation defects on the GNAS locus, aiming to clarify clinical characteristics within different patient groups.
  • Researchers analyzed 84 patients, categorizing them into five groups based on their methylation patterns and inheritance types, looking at symptoms and ages at diagnosis.
  • Results showed that the sporadic PHP1B group had younger patients and distinct symptoms compared to others, and while neurodevelopmental disorders were noted, no clear links were found between methylation ratios and hormone levels.

Biomarkers are an important drug developmental tool. Assessment of quantitative analytical methods of biomarkers is not included in any regulatory documents in Japan. Use of biomarkers in clinical evaluations and supporting the post-marketing evaluation of drug efficacy and/or adverse reactions requires assessment and full validation of analytical methods for these biomarkers.

Studies show that the burden of caregiving tends to fall on individuals of low socioeconomic status (SES); however, the association between SES and the likelihood of caregiving has not yet been established. We studied the relationship between SES and the likelihood of adults providing long-term care for their parents in Japan, where compulsory public long-term insurance has been implemented. We used the following six comprehensive measures of SES for the analysis: income, financial assets, expenditure, living conditions, housing conditions, and education.

  • Atrophic autoimmune thyroiditis (AAT) is an uncommon form of hypothyroidism in children, characterized by the absence of goiter and presence of TSH receptor-blocking antibodies.
  • This study aimed to determine the prevalence of these antibodies in children diagnosed with AAT by reviewing data from patients under 15 years old.
  • The results showed that 38.8% of the 18 patients tested positive for TSH receptor antibodies, suggesting that AAT may be more prevalent in children than previously thought.

Although Turner syndrome (TS) is frequently associated with congenital anomalies of the kidney-urinary tract (CAKUT), which is a major cause of pediatric chronic kidney disease, renal function in TS is usually considered normal. The present study aimed to analyze the frequency of renal dysfunction and CAKUT in pediatric patients with TS. Our study included 122 patients with TS between the ages of 2 and 18 years from 30 hospitals across Japan.

Hyperammonemia is a typical symptom of urea cycle disorders. While early-onset argininosuccinic aciduria (ASA) can often be detected by hyperammonemia, patients with late-onset ASA predominantly present with psychomotor retardation and mental disorders. However, in late-onset ASA that develops during early childhood, hyperammonemia can sometimes be caused by acute infections, stress, and reduced dietary intake.

Type 1 diabetes (T1D) is an autoimmune disease in which insulin-producing pancreatic β-cells are destroyed. Intestinal helminths can cause asymptomatic chronic and immunosuppressive infections and suppress disease in rodent models of T1D. However, the underlying regulatory mechanisms for this protection are unclear.
