Immunosuppressive maintenance therapy after kidney transplantation leads to various undesired side effects such as calcineurin inhibitor (CNI)-associated nephrotoxicity or elevated cardiovascular risk due to posttransplantation diabetes and hypertension. These effects show negative impacts on long-term allograft function as well as patient morbidity and mortality. Therefore, we used an immunosuppressive regimen with early corticosteroid withdrawal (ESW), maintenance therapy containing tacrolimus, sirolimus (SRL), and mycophenolate sodium for 3 months followed by a prospective randomized trial comparing a CNI free versus a low-dose CNI therapy.

Background: Little is known about the safety of steroid withdrawal after ABO blood group-incompatible living donor kidney transplantation.

Methods: Between September 2005 and November 2007, a total of 15 patients were successfully transplanted over the blood group barrier in our transplantation centers. Similarly to transplant recipients with normal immunological risk, we aimed to taper and eventually stop oral prednisone because of the well-known negative impact of steroids on cardiovascular morbidity and mortality, which in turn is limiting graft survival.

For patients with end-stage renal disease and hereditary haemochromatosis, prevention and treatment of anaemia differ from usual nephrologic guidelines. Monitoring of individual disease progression and ferritin levels is crucial. We describe a case of a young haemodialysis patient with early-stage organ dysfunction caused by hereditary haemochromatosis, in whom iron stores have successfully been depleted with phlebotomy and supplemental erythropoietin over 22 months.

Local inflammation and systemic symptoms such as fever, fatigue and loss of appetite suggest the diagnosis of an abscess even if the patient is in a good general physical condition. The site of bacterial penetration should be identified along the corresponding lymphatic vessels. In this case, periauricular scratching induced by severe psoriasis led to a cervical abscess, the causing bacteria were identified as Streptococcus pyogenes.

We diagnosed an aggressive Non-Hodgkin-Lymphoma (diffuse large B-cell lymphoma, DLBCL) in a patient with a fast growing inguinal mass. Only one third of all suspicious inguinal lymph nodes that are investigated histologically are found to be malignant lymphoma. If so, an accurate histopathological evaluation of sufficient neoplastic tissue is critical.
