The Effectiveness of Nurse-Led Transition Care on Post-Discharge Outcomes of Adult Stroke Survivors: A Systematic Review and Meta-Analysis.

Aim: To evaluate the effectiveness of nurse-led Transition Care (TC) on quality of life, mortality and readmission among adult stroke survivors.

Design: Systematic review and meta-analysis using the Joanna Briggs Institute (JBI) methodology for systematic reviews of effectiveness.

Methods: Databases were searched from 1st January 2000 to 31st December 2023.

Perceptions and responses to cognitive decline in people with diabetes: A systematic review of qualitative studies.

Objective: We aimed at summarizing the perceptions and responses to cognitive decline, assessing the disease management, identifying deficiencies and proposing new strategies for improvement in people with diabetes (PWDs).

Methods: A comprehensive search was performed in the following nine databases: PubMed, EMBASE, Web of Science, The Cochrane Library, PsycINFO, CINAHL, WanFang, CNKI, and VIP. The Joanna Briggs Institute (JBI) Critical Appraisal Tool for qualitative research was utilized to evaluate the quality of included studies.
