Precis: Second trabeculectomy and glaucoma drainage device implantation offer similar success rates for juvenile open angle glaucoma after initial trabeculectomy failure. However, second trabeculectomies required fewer medications. A quarter of patients in both groups required a third operation after 2.

Article Synopsis
  • The study aimed to assess how effective manual grading of anterior segment optical coherence tomography (AS-OCT) is in detecting plateau iris configuration (PIC) and comparing it with established ultrasound biomicroscopy (UBM) criteria.
  • A total of 179 eyes from 142 patients were analyzed, revealing that only about 47.5% had a confirmed diagnosis of PIC, with varying sensitivity (56-78%) and specificity (49-64%) in AS-OCT for diagnosing PIC.
  • The findings concluded that manual grading of AS-OCT for detecting PIC is not adequately reliable to replace UBM, as the accuracy rates were relatively low and the agreement between AS-OCT and UBM was only fair.

Article Synopsis
  • - The study aimed to assess how effective deep learning (DL) algorithms are in predicting plateau iris using anterior segment optical coherence tomography (AS-OCT) images, particularly for patients with primary angle closure disease (PACD).
  • - Researchers collaborated with a tech department to develop and validate the DL model, analyzing data from 179 eyes and comparing the algorithm's predictions against ultrasound biomicroscopy (UBM) findings.
  • - Results showed the DL model achieved high accuracy with a sensitivity of 87.9%, specificity of 97.6%, and an area under the curve (AUC-ROC) of 0.95, indicating strong potential for aiding clinicians in diagnosing this condition.

Purpose: To evaluate the long-term outcomes of second Ahmed glaucoma valve (AGV) implants in eyes with glaucoma.

Design: Retrospective interventional case series.

Methods: Patients with sequential second Ahmed valves implanted in the same eye from 1994 to 2016 were included.

Purpose: To present a digital image subtraction technique to alert clinicians to signs of glaucomatous optic disc progression.

Methods: Ninety-two glaucomatous eyes (65 patients) were included. Thirty-three eyes were identified as progressive and 59 as stable based on comparison of baseline and follow-up stereoscopic disc photographs by three masked glaucoma specialists.
