Background: Several randomized clinical trials have shown that the composite endpoint of death, stroke, and myocardial infarction (MI) is equivalent between carotid artery stenting and carotid endarterectomy. However, the risk of minor stroke has been consistently higher with carotid artery stenting.

Objectives: The authors sought to evaluate the safety and effectiveness of a novel carotid stent system comprised of a stent, an adjustable integrated embolic filter and a postdilation balloon, in patients at elevated risk for adverse events from carotid endarterectomy.

Article Synopsis
  • Using MRI-fusion guided prostate biopsy (PB) leads to better agreement with prostatectomy histopathology results compared to traditional 12-core TRUS biopsy, especially in patients with higher-risk prostate cancer.
  • In a study involving 218 men diagnosed with prostate cancer, those who underwent MRI-fusion PB had a significantly higher concordance rate (73.1% vs. 42.9%) with the final pathology from surgery.
  • The research highlighted that the number of cancer-involved regions in the prostate is an important factor influencing concordance, indicating that MRI-fusion techniques enhance diagnostic accuracy and treatment planning for patients.

Importance: Much remains unknown about the extent of and factors that influence clinician-level variation in rates of admission from the emergency department (ED). In particular, emergency clinician risk tolerance is a potentially important attribute, but it is not well defined in terms of its association with the decision to admit.

Objective: To further characterize this variation in rates of admission from the ED and to determine whether clinician risk attitudes are associated with the propensity to admit.

  • The study examined how older men and women are treated for heart attacks (STEMI and NSTEMI) in six countries, focusing on hospitalization rates, intervention procedures, and outcomes from 2011 to 2018.
  • Results showed that while hospitalization rates for heart attacks fell across all countries, the rate ratio of male to female hospitalizations rose, indicating a growing disparity.
  • Females consistently received fewer interventional procedures than males for STEMI across the board, and mortality rates differed, with females often faring worse in STEMI outcomes in most countries but better in NSTEMI outcomes in others.

  • Total body computerized tomography (TBCT) is often used for diagnosing fever of unknown origin (FUO), but this study evaluated its effectiveness compared to the recommended FDG-PET/CT method.
  • Among the 408 patients studied, 40.2% had positive TBCT results, primarily identifying infections (58.5%), neoplasms (22.8%), and inflammatory disorders (14.0%), with chest and abdomen being the most affected areas.
  • The study found that TBCT was particularly effective in patients with elevated CRP levels, low hemoglobin, and high white blood cell counts, suggesting that TBCT can be a valuable diagnostic tool when FDG-PET/CT is not available.

Portal hypertension often precedes the development of advanced fibrosis in patients with Metabolic dysfunction-associated steatotic liver disease (MASLD) and may accelerate disease progression to cirrhosis. We aimed to evaluate whether prioritization tools accurately predict survival in patients with MASLD and clinically significant portal hypertension (CSPH). We retrospectively identified patients diagnosed with esophageal or gastric varices (EGV).

Article Synopsis
  • - The study investigates caregiver motivations in treating younger patients with septic shock in the ICU, focusing on whether they achieve unnecessarily high mean arterial pressure (MAP) values.
  • - A retrospective analysis of 555 patients from 2006 to 2018 revealed that younger patients (ages 18-45) had average MAP values that were 4.7 mmHg higher than those of patients older than 75, even after controlling for other factors.
  • - Findings suggest that ICU staff may unconsciously adjust noradrenaline treatment to achieve higher MAP targets for younger patients, despite no evidence that this approach is more beneficial compared to older patients.

Purpose: To study the experiences of patients with hepatocellular carcinoma (HCC) contributing to treatment discrepancy in the United States.

Materials And Methods: Using Surveillance, Epidemiology, and End Results data from National Cancer Institute (NCI), Medicare (2002-2015) beneficiaries with HCC who completed a Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey were included. Six CAHPS items (3 global scores: global care rating [GCR], primary doctor rating [PDR], and specialist rating [SR]; 3 composite scores: getting needed care [GNC], getting care quickly [GCQ], and doctor communication [DC]) assessed patient experience.

Objectives: Reducing medical practice variation (MPV) is a central theme of system improvement because it is associated with poor health outcomes, increased costs and disparities in care. This study aimed to estimate the extent to which each determinant (patient, physician, clinic) explains MPV among primary care physicians and to identify the characteristics of health services with a greater explained variance.

Methods: A retrospective cohort study of primary care physicians practising in non-private clinics of Clalit Health Services in Southern Israel, for longer than a year between 2011 and 2017 and with more than 100 adult patients per practice.

Background: Hip fractures are costly and common in older adults, but there is limited understanding of how treatment patterns and outcomes might differ between countries.

Methods: We performed a retrospective serial cross-sectional cohort study of adults aged ≥66 years hospitalized with hip fracture between 2011 and 2018 in the US, Canada, England, the Netherlands, Taiwan, and Israel using population-representative administrative data. We examined mortality, hip fracture treatment approaches (total hip arthroplasty [THA], hemiarthroplasty [HA], internal fixation [IF], and nonoperative), and health system performance measures, including hospital length of stay (LOS), 30-day readmission rates, and time-to-surgery.

Vaccination, especially with multiple doses, provides substantial population-level protection against COVID-19, but emerging variants of concern (VOC) and waning immunity represent significant risks at the individual level. Here we identify correlates of protection (COP) in a multicenter prospective study following 607 healthy individuals who received three doses of the Pfizer-BNT162b2 vaccine approximately six months prior to enrollment. We compared 242 individuals who received a fourth dose to 365 who did not.

Background: Targeting biopsy (TBx) of suspicious lesions combined with random systematic biopsy (SBx) improves detection rates of prostate cancer (PCa) during magnetic resonance imaging (MRI)/ultrasound (US) fusion prostate biopsy. However, this combination increases the number of biopsy cores, prolongs the procedure time, and increases complications and costs, leading to the overdiagnosis of clinically insignificant PCa (ciPCa). This study aims to evaluate the optimal sampling design to achieve a detection rate of clinically significant PCa (csPCa) equal to standard TBx with SBx with fewer biopsy cores.
