Article Synopsis
  • - This study examined the outcomes of elderly patients (over 65 years) with gastric cancer who received best supportive care, focusing on those diagnosed between 2014 and 2019.
  • - Results showed that median survival times varied significantly based on cancer stage, with stage 0 patients surviving an average of 1,987 days compared to only 43 days for stage IV patients.
  • - The findings indicated that cancer stage and performance status are important factors impacting both overall and cancer-specific mortality in elderly patients, helping guide treatment decisions in future cases.

We report a rare case of undifferentiated-type intramucosal gastric cancer that occurred in the fornix of the stomach without Helicobacter pylori infection, which consisted mainly of poorly differentiated adenocarcinoma. A 49-year-old man visited our hospital for a follow-up endoscopic examination of a small depressed lesion of the gastric fornix detected by surveillance esophagogastroduodenoscopy. On magnifying endoscopy with blue laser imaging, the depressed lesion (approximately 10 mm in diameter) was regarded as undifferentiated-type early gastric cancer that proved to be a poorly differentiated adenocarcinoma by histological examination of biopsied specimens.

Article Synopsis
  • - A patient with liver dysfunction was diagnosed with IgG4-related autoimmune hepatitis (IgG4-AIH) after a liver biopsy revealed IgG4-positive plasma cell infiltration and high serum IgG4 levels.
  • - Later imaging studies showed the presence of two inflammatory pseudotumors (IPTs) in the liver, which are also considered IgG4-related diseases, marking a rare concurrent occurrence of both conditions.
  • - The patient was treated with steroids for a separate diagnosis of cryptogenic organized pneumonia, leading to improvement in her liver dysfunction and resolution of one of the IPTs.

A 41-year-old woman who had a pancreatic tail tumor and multiple liver tumors was referred to our hospital. The results of abdominal US, CT and MRI, and histopathological and immunohistochemical findings of the liver tumor biopsy revealed a pancreatic neuroendocrine tumor with excessively-advanced liver metastasis. We treated her with S-1/gemcitabine combination chemotherapy plus long-acting somatostatin analogue octreotide, which produced tumor stabilization and good quality of life for 7 months, and survival time of 15 months.

We conducted a drastic change in our hospital information system to support patient participatory health care provided in the New Hiroshima University Hospital (HUH). The new information system in HUH (HU-MIND II-Hiroshima University Hospital Medical Intelligence and Notice Delivery system II) is designed as "clinical management system" (CMS). The core of this CMS is the electronic health record (EHR), which aims to assure both the patients' right to know, and the attendants' accountability.
