Article Synopsis
  • - TEVAR is a common treatment for complicated acute and high-risk uncomplicated Stanford Type-B aortic dissections, redirecting blood flow to seal the dissection tear, yet over 30% of patients still need more treatments due to ongoing issues with the false lumen.
  • - Researchers propose a new technique called slit fenestration pattern creation to lessen the forces needed to keep the dissection flap in place, which could help in reducing complications post-TEVAR.
  • - The effectiveness of this technique has been shown through virtual experiments and benchtop testing using porcine aortas, indicating it can significantly lower the pressure required for proper aortic wall remodeling.

Net energy (NE) enables the prediction of more accurate feed energy values by taking into account the heat increment which is approximately 25% of apparent metabolizable energy (AME) in poultry. Nevertheless, application of NE in poultry industry has not been practiced widely. To predict the NE values of broiler diets, 23 diets were prepared by using 13 major ingredients (wheat, corn, paddy rice, broken rice, cassava pellets, full-fat soybean, soybean meal, canola meal, animal protein, rice bran, wheat bran, palm kernel meal and palm kernel oil).

Different energy systems have been proposed for energy evaluation of feeds for domestic animals. The oldest and most commonly used systems take into account the fecal energy loss to obtain digestible energy (DE), and fecal, urinary and fermentation gases energy losses to calculate metabolizable energy (ME). In the case of ruminants and pigs, the net energy (NE) system, which takes into account the heat increment associated with the metabolic utilization of ME, has progressively replaced the DE and ME systems over the last 50 years.

Article Synopsis
  • - Current treatments for diverticular disease can't stop future issues like diverticulitis or bleeding, highlighting the need for new methods to manage the condition.
  • - Researchers introduced a novel adhesive patch to mechanically support the affected colon, testing its effectiveness in a pig model to evaluate safety and performance.
  • - After 3 months, results showed that the patch significantly reduced pouch deformation and improved collagen levels in the colon tissue, suggesting it could help prevent complications associated with diverticulosis.

The cost of feed represents an important part of the total cost in swine and poultry production (>60%) with energy accounting for at least 70% of feed cost. The energy value of ingredients or compound feeds can be estimated as digestible (DE), metabolisable (ME) and net energy (NE) in pigs and ME and NE in poultry. The current paper reviews the different methods for evaluating DE, ME and NE of feeds for monogastric animals and their difficulties and limits, with a focus on NE.

Background: Key challenges to the successful conduct of The Emergency treatment with Levetiracetam or Phenytoin in Status Epilepticus in children (EcLiPSE) trial were identified at the pre-trial stage. These included practitioner anxieties about conducting research without prior consent (RWPC), inexperience in conducting an ED-led trial and use of a medication that was not usual ED practice. As part of an embedded study, we explored parent and practitioner experiences of recruitment, RWPC and conduct of the trial to inform the design and conduct of future ED-led trials.

Background: Alternatives to prospective informed consent enable the conduct of paediatric emergency and critical care trials. Research without prior consent (RWPC) involves practitioners approaching parents after an intervention has been given and seeking consent for their child to continue in the trial. As part of an embedded study in the 'Emergency treatment with Levetiracetam or Phenytoin in Status Epilepticus in children' (EcLiPSE) trial, we explored how practitioners described the trial and RWPC during recruitment discussions, and how well this information was understood by parents.

We present a patient with a ruptured basilar tip aneurysm treated under conscious sedation via a right radial approach using the Woven EndoBridge device. We discuss the technical aspects of the case as the well as the available literature on the treatment of ruptured aneurysms treated without general anaesthesia. We discuss the potential benefits of the Woven EndoBridge device in such situations.

Diverticulosis results from the development of pouch-like structures, called diverticula, over the colon. The etiology of the disease is poorly understood resulting in a lack of effective treatment approaches. It is well known that mechanical stress plays a major role in tissue remodeling, yet its role in diverticulosis has not been studied.

The goal of this study was to characterize the thoracic duct (TD) both morphologically and hemodynamically. The lymphatic flow and pressure gradient from the cisterna chyli (CC) to the lymphovenous junction were measured in anesthetized swine ( = 9). After the animals were euthanized, the TD were harvested for histomorphometric analyses in which three samples were perfused with 9% gelatin to obtain the morphometry of the TD valve in both the open and closed configuration.

This study tested the hypothesis that (pyr)apelin-13 dose-dependently augments myocardial contractility and coronary blood flow, irrespective of changes in systemic hemodynamics. Acute effects of intravenous (pyr)apelin-13 administration (10 to 1,000 nM) on blood pressure, heart rate, left ventricular pressure and volume, and coronary parameters were measured in dogs and pigs. Administration of (pyr)apelin-13 did not influence blood pressure ( = 0.

Using accurate nutrient values for ingredients is of vital importance for efficient diet formulation. The net energy (NE) system accounts for the real available amount of feedstuff energy for body maintenance and production as it considers energy dissipated as heat increment. The NE content of diets for pigs and broilers has been estimated from their nutrient contents.

The aim of this study was to evaluate the associated effects of ambient temperature and inflammation caused by repeated administration of Escherichia coli lipopolysaccharide (LPS) on insulin, energy, and AA metabolism. Twenty-eight pigs were assigned to one of the two thermal conditions: thermoneutral (24 °C) or high ambient temperature (30 °C). The experimental period lasted 17 d, which was divided into a 7-d period without LPS (days -7 to -1), and a subsequent 10-d LPS period (days 1 to 10) in which pigs were administered 5 repeated injections of LPS at 2-d intervals.

Background: Phenytoin is the recommended second-line intravenous anticonvulsant for treatment of paediatric convulsive status epilepticus in the UK; however, some evidence suggests that levetiracetam could be an effective and safer alternative. This trial compared the efficacy and safety of phenytoin and levetiracetam for second-line management of paediatric convulsive status epilepticus.

Methods: This open-label, randomised clinical trial was undertaken at 30 UK emergency departments at secondary and tertiary care centres.

Background: EcLiPSE (Emergency treatment with Levetiracetam or Phenytoin in Status Epilepticus in children) is a randomised controlled trial (RCT) in the United Kingdom. Challenges to success include the need to immediately administer an intervention without informed consent and changes in staffing during trial conduct, mainly due to physician rotations. Using literature on parents' perspectives and research without prior consent (RWPC) guidance, we developed an interactive training package (including videos, simulation and question and answer sessions) and evaluated its dissemination and impact upon on practitioners' confidence in recruitment and consent.

Aortic dissection (AD) involves tearing of the medial layer, creating a blood-filled channel called false lumen (FL). To treat dissections, clinicians are using endovascular therapy using stent grafts to seal the FL. This procedure has been successful in reducing mortality but has failed in completely re-attaching the torn intimal layer.

Global consumption of chicken meat has increased at a faster rate than any other animal protein source, and thus refinements in energy formulation techniques for feed have continued to gain importance. Formulation of animal feed based on net energy (NE) has been implemented in ruminants and pigs but not in poultry. A closed-circuit respiratory calorimetry system was employed on 25- to 28-day-old broilers fed 19 diets formulated with varying nutrient composition to produce equations to predict NE and apparent metabolizable energy (AME) efficiency of feed for broiler chickens.

The use of endovascular treatment in the thoracic aorta has revolutionized the clinical approach for treating Stanford type B aortic dissection. The endograft procedure is a minimally invasive alternative to traditional surgery for the management of complicated type-B patients. The endograft is first deployed to exclude the proximal entry tear to redirect blood flow toward the true lumen and then a stent graft is used to push the intimal flap against the false lumen (FL) wall such that the aorta is reconstituted by sealing the FL.

Diverticulosis is a structural alteration of the colon tissue characterized by the development of pouch-like structures called diverticula. It afflicts a significant portion of the population in Western countries, with a higher prevalence among the elderly. Diverticulosis is believed to be the result of a synergetic interaction between inherent tissue weakness, diet, colonic microstructure, motility, and genetic factors.
