Effectiveness of Chlorhexidine vaginal cleansing in reducing post-caesarean endometritis at two tertiary hospitals in Enugu, Nigeria: A randomized controlled trial.

Despite the use of prophylactic antibiotics during Caesarean delivery, post-Caesarean surgical site infection (SSI) and endometritis still often complicate the puerperal period. We speculated that the use of vaginal cleansing with chlorhexidine solution would serve as an adjunct to prophylactic antibiotics in reducing the menace of post-caesarean endometritis and indeed found a significant reduction.

Septic arthritis of the acromioclavicular joint secondary to methicillin-resistant infection.

Septic arthritis of the acromioclavicular joint (ACJ) is a rare pathology with very few documented case reports available in the literature. In addition, ACJ septic arthritis secondary to methicillin-resistant (MRSA) is a rarer pathology with only two previously documented case reports in the medical literature to our knowledge. We present the case of a 46-year-old man who was diagnosed with a confirmed ACJ septic arthritis secondary to MRSA following admission to the local trauma and orthopaedic service at a district general hospital.

Sequential bilateral lesser trochanter avulsion fractures in an adolescent patient.

We report the rare case of a 15-year-old boy who sustained sequential bilateral lesser trochanter avulsion fractures over a 6-month period while playing football. No malignancy or associated metabolic bone disease was found. He subsequently made a full recovery with non-operative treatment.

Nonoperative treatment of humeral shaft fractures revisited.

Purpose: The purpose of this study was to examine the union rate of humeral shaft fractures treated nonoperatively and to establish whether a particular fracture type is more likely to go on to nonunion.

Methods: Radiographs and patient records of 207 humeral shaft fractures occurring during 5 years were retrospectively reviewed. All patients were initially managed nonoperatively and placed in a U-slab on diagnosis in the emergency department; this was converted to a functional humeral brace at 7 to 10 days after injury.

Changing transfusion practice in total hip arthroplasty: observational study of the reduction of blood use over 6 years.

Patients undergoing primary total hip arthroplasty (THA) have historically been over-transfused. In a district general hospital setting, the authors observed a significant downward trend in blood transfusion requirements in these patients over 6 years after a change in transfusion policy. The purpose of this study was to retrospectively analyze the change in transfusion practice and present the results of the restrictive transfusion policy.
