Cardiovascular diseases are the leading cause of death among women, and the incidence among younger women has shown the greatest increase over the last decades, in particular for acute myocardial infarction (AMI). Moreover, the prognosis of women post-AMI is poor when compared with men of similar ages. Since the 1990s, an abundant literature has highlighted the existing differences between sexes with regard to presentation, burden, and impact of traditional risk factors and of risk factors pertaining predominantly to women, the perception of risk by women and men, and the pathophysiological causations, their treatment, and prognosis.

Introduction: Neurological disorders (ND) have a high incidence in sub-Saharan Africa (SSA). In this region, systemic challenges of conventional medicine (CM) and cultural beliefs have contributed to a large utilization of traditional medicine (TM). Yet, data on TM and those who use it in the treatment of ND in SSA are scarce.

Background: Patient decision-making autonomy refers to the patients' ability to freely exert their own choices and make their own decisions, given sufficient resources and information to do so. In pain medicine, it is accepted that appropriate beneficial management aims to propose an individualized treatment plan shared with the patients, as agents, to help them live as autonomously as possible with their pain. However, are patients in chronic pain centers sufficiently autonomous to participate in the therapeutic decisions that concern them? As this question still remains unanswered, a pilot study was set up to that aim.

Background: Long-term use of opioids does not result in significant clinical improvement and has shown more adverse than beneficial effects in chronic pain conditions. When opioids cause more adverse effects than benefits for the patient, it may be necessary to initiate a process of deprescribing.

Aim: To explore the perceptions of French pain physicians regarding the process of opioid deprescribing in patients experiencing chronic non-cancer and to generate an understanding of the barriers and levers to the deprescribing process.

Article Synopsis
  • Arrhythmic mitral valve prolapse (AMVP) is a serious heart condition that can cause dangerous heart rhythms, especially in young women.
  • Researchers looked at women with AMVP who had troubling heart rhythms during and after pregnancy to see if being pregnant raised their risk.
  • They found that these women had more dangerous heart rhythms during pregnancy and up to 6 months after giving birth compared to when they weren't pregnant.

Background: It is currently considered that around 30% of chronic pain patients are totally refractory to medical treatment. Among patients who remain responsive to medical treatment, it is estimated that between 20% and 50% are likely to discontinue treatment due to severe side effects. Given these therapeutic difficulties, a significant number of patients turn to complementary therapies.

Aims: Inherited cardiomyopathies are relatively rare but carry a high risk of cardiac maternal morbidity and mortality during pregnancy and postpartum. However, data for risk stratification are scarce. The new CARPREG II score improves prediction of prognosis in pregnancies associated with heart disease, though its role in inherited cardiomyopathies is unclear.

Context: In addition to curative care, supportive care is beneficial in managing the anxiety symptoms common in patients in sterile hematology unit. We hypothesize that personal massage can help the patient, particularly in this isolated setting where physical contact is extremely limited. The main objective of this study was to show that anxiety could be reduced after a touch-massage® performed by a nurse trained in this therapy.

Article Synopsis
  • - The study focuses on continuous and deep sedation maintained until death (CDSUD) in palliative care, especially for patients experiencing refractory suffering or who request to stop life-sustaining treatment.
  • - A retrospective analysis of 42 hospitalized patients showed that 79% initiated CDSUD due to persistent suffering, with midazolam as the primary sedative, often combined with chlorpromazine and propofol.
  • - Findings reveal an average sedation duration of 37 hours, with specific dosages for each sedative, contributing valuable insights into CDSUD practices in palliative care settings.

Background: Pain is a common symptom in palliative care cancer patients and is often insufficiently relieved. In recent years, transcranial direct-current stimulation (tDCS) of the motor cortex has been shown to be effective to treat chronic pain, essentially neuropathic pain. We propose to test the efficacy of tDCS in patients experiencing cancer pain in the palliative care setting.

Article Synopsis
  • The study investigates the outcomes of critically ill pregnant women with severe ARDS who underwent ECMO, focusing on whether the timing of delivery (before or after ECMO) affects maternal and fetal health.
  • Out of 563 women, those whose delivery was performed after ECMO faced higher rates of major bleeding, while fetal survival rates were significantly better when ECMO was started post-delivery.
  • Despite risks, overall maternal survival rates were high, and newborns showed no severe complications or long-term health issues, indicating the need for careful decision-making regarding delivery timing in these critical cases.

Background: In pediatric intensive care units (PICUs), parents and healthcare professionals attend to children who verbally and non-verbally express their pain and suffering, fears, anxieties, desires, and wishes in complex intensive care situations. What can we learn from these experiences to improve the way we can take care of and support children?

Objectives: The main objective of this clinical ethics study was to focus on the experience stories of parents during their child's hospitalization in a PICU, to analyze their discourse, and to propose an ethical perspective.

Method: The current research collects the experience reports of parents during their child's hospitalization in a PICU and those of the caregivers who treated them.
