Serum alpha-1 proteinase inhibitor in advanced cancer: mass variants and functionally inert forms.

In 1984, we reported that while immunoreactive levels of serum alpha-1 proteinase inhibitor (API) increased significantly in nine patients with advanced solid tumors, the functional activity of the inhibitor, as measured by the serum trypsin inhibitory capacity, did not increase proportionately. This suggested that a portion of the circulating API was functionally inert. We have now assayed immunoreactive titers and trypsin inhibitory capacity of serum API of 49 patients with advanced carcinomas and 27 healthy controls.

A randomized study of oral nutritional support versus ad lib nutritional intake during chemotherapy for advanced colorectal and non-small-cell lung cancer.

One hundred ninety-two patients with previously untreated metastatic cancer (102 non-small-cell lung cancer [NSCLC]; 90 colorectal cancer) were randomized to receive either ad lib nutritional intake (control group) or specific nutritional intervention during a 12-week study period when chemotherapy was administered. Those patients randomized to nutritional interventions were counselled to take oral nutrients with caloric intake equal to 1.7 to 1.

Indomethacin in streptozocin-induced nephrogenic diabetes insipidus.

A 14-year-old female patient with metastatic carcinoid developed streptozocin-induced glomerular, proximal, and distal tubular dysfunction. The latter was in the form of nephrogenic diabetes insipidus, with urine volumes in excess of 11 L/24 h. The prostaglandin synthetase inhibitor, indomethacin, rapidly corrected the polyuria both initially and on rechallenge, independent of change in glomerular filtration rate.

Nutritional support in chemotherapy and radiation therapy patients is a very attractive concept. This group of patients frequently begins treatment malnourished, and the malnutrition becomes worse because of treatment-related nausea and vomiting, mucositis, fever and other side effects. Effective nutritional support would enhance wound healing and visceral function, improve cellular immunity and allow delivery of adequate drug and irradiation doses, thereby increasing response rates and prolonging survival.

We describe an inductively coupled plasma atomic emission spectrometer that has been adapted to perform routine, simultaneous, direct analyses of calcium, magnesium, copper, zinc, and iron in serum or urine without sample digestion or pretreatment. The system, constructed with inexpensive, readily available components, can analyze 1-mL or smaller samples. Results correlate nearly perfectly with those derived by standard atomic absorption techniques (r = 0.

Conversion of CSF monomeric growth hormone to large growth hormone with exposure to serum.

We have found a dissociation between CSF and serum growth hormone heterogeneity in a patient with suprasellar extension of a growth hormone-secreting pituitary tumour. When CSF was studied using gel chromatography, virtually all the growth hormone eluted as monomeric growth hormone with only 2.4% eluting before albumin as large growth hormone ('big, big' growth hormone).

Immunoglobulins associated with elevated riboflavin binding by plasma from cancer patients.

Basal energy expenditure accounts for a large component of energy losses, and a clinical estimate of this form of thermogenesis is usually derived from a prediction equation. Characterization of autostimulatory and transforming growth factors from human melanoma cells.

Serum-free growth of the human malignant melanoma cell line Hs0294 is associated with production of transforming growth factor-alpha and an autostimulatory melanoma mitogen (melanoma growth-stimulatory activity, MGSA). The transforming activity is characterized by stimulation of anchorage-independent growth of normal rat kidney fibroblasts and competition with 125I-epidermal growth factor for binding to normal rat kidney cells. The second activity, MGSA, stimulates the anchorage-dependent growth of human melanoma cells in serum-free culture medium.

The effect of a phenylalanine and tyrosine restricted diet on elemental balance studies and plasma aminograms of patients with disseminated malignant melanoma.

Previous studies indicate dietary phenylalanine and tyrosine restriction may be of value in managing advanced cancer patients. To further evaluate this approach, we performed a 60-day study in which four patients with advanced malignant melanoma received formula diets via nasogastric tube containing only 8 mg total phenylalanine and tyrosine per kg lean body mass per day. Two of three patients completing elemental balance studies were in negative nitrogen, potassium, and phosphorus balance, suggesting an essential nutrient deficiency.

Bioenergetic studies in adult patients recovering from semi-starvation.

A model of energy-nitrogen balance developed in animals was adapted to the study of human bioenergetics. By creating balance conditions and steady-state formula infusion rates, each of the needed components of energy and nitrogen balance can now be measured with a high level of accuracy. A whole body gradient-layer calorimeter was designed for thermophysiologic studies in hospitalized patients. The instrument was assembled in a room adjoining a metabolic ward. The patient lies recumbent on a stretcher which is wheeled into the calorimeter on tracks.

Structures of the oligosaccharide chains of two forms of alpha 1-acid glycoprotein purified from liver metastases of lung, colon, and breast tumors.

Two forms of alpha 1-acid glycoprotein with common immunological determinants and almost identical amino acid compositions but different amounts of carbohydrate were isolated from liver metastases of primary colon, lung, and breast tumors by extraction with perchloric acid, gel filtration on Sepharose CL-6B and Sephadex G-200, and affinity chromatography on concanavalin A:agarose and Ricinus communis agglutinin l:agarose. Both forms of the antigen yielded single bands which stained for protein and carbohydrate when examined by disc gel electrophoresis and immunodiffusion. The molecular weights of the two forms were 45,000 and 37,000 respectively.

Treatment of small cell carcinoma of the lung with COPP.

The COPP regimen (cyclophosphamide, Oncovin, procarbazine, prednisone) was one of the first chemotherapy programs to prolong survival in small cell lung cancer (SCLC) significantly. Results with this regimen in 50 patients with SCLC show an overall median survival of 10 months. Survival was decreased with more extensive disease.
