A 17-year-old male diagnosed with systemic lupus erythematosus (SLE), showing poor compliance with medication, presented to our facility with a 20-day history of fever, polyarthritis, and cough. Additionally, he had experienced a seizure episode, followed by a one-day history of altered mentation. Subsequently, he developed pneumonia, respiratory distress, and shock, necessitating ventilator and inotropic support.

The common pathogen encountered in sepsis with asplenic/hyposplenism patients is encapsulated organism, gram-negative bacilli, but in developing countries like India, there are many possibilities of infection by an uncommon organism that make it difficult to diagnose. Here, we have described a case of sickle cell disease presented with persistent fever and later, found to have an atrophic spleen with involvement of respiratory system and osteoarticular system.

Infectious spleen and kidney necrosis virus (ISKNV), a member of family iridoviridae, reported for the first time in a wide range of ornamental fish species in India. Significant mortalities during the year 2018-19 were reported from a number of retailers in the region with various clinical signs. The samples of moribund, dead and apparently healthy ornamental fishes were collected from retailers, located in three districts of Karnataka, India.

Our laboratory reported the first case of RSIV infection in India which resulted in mass mortalities of Asian seabass, Lates calcarifer.
