A Large Pelvic Haematoma Caused by a Local Anaesthetic Flexible Cystoscopy Injection of Intradetrusor Botulinum Toxin A: A Case Report.

A 55-year-old female attended the Outpatient Urology Department for local anaesthetic flexible cystoscopy and intradetrusor botulinum toxin A injection. Having been diagnosed with urodynamics-proven low-grade detrusor overactivity in 2017, she was well-established on six-monthly Botox® injections. As part of her ongoing treatment, 100 units of Allergan Botox diluted with saline in a 10 mL syringe were injected via 20 punctures.

Class I lysine deacetylases promote glucocorticoid-induced transcriptional repression through functional interaction with LSD1.

Small molecule inhibitors of lysine deacetylases (KDACs) are approved for clinical use in treatment of several diseases. Nuclear receptors, such as the glucocorticoid receptor (GR) use lysine acetyltransferases (KATs or HATs) and KDACs to regulate transcription through acetylation and deacetylation of protein targets such as histones. Previously we have shown that KDAC1 activity facilitates GR-activated transcription at about half of all cellular target genes.

A UK perspective on smartphone use amongst doctors within the surgical profession.

Introduction: Hospitals are increasingly looking for mobile solutions to meet their information technology needs. Medical professionals are using personal mobile devices to support their work, because of limitations in both time and space. Our aims were to assess smartphone use amongst UK surgical doctors, the prevalence of medical app use and online activity.

Nuclear receptors use lysine acetyltransferases and lysine deacetylases (KDACs) in regulating transcription through histone acetylation. Lysine acetyltransferases interact with steroid receptors upon binding of an agonist and are recruited to target genes. KDACs have been shown to interact with steroid receptors upon binding to an antagonist.

A right inguinoscrotal swelling: the importance of histological analysis.

Myxofibrosarcoma of the spermatic cord is a rare condition. We present an unusual case of a 69-year-old gentleman who presented to the emergency department on two occasions with a right inguinoscrotal swelling. We will explore the diagnostic difficulties encountered and subsequent management.

Synthesis of Tetrazolo-Fused Benzodiazepines and Benzodiazepinones by a Two-Step Protocol Using an Ugi-Azide Reaction for Initial Diversity Generation.

A two-step strategy for the synthesis of arrays of tricyclic tetrazolo-fused benzodiazepines and benzodiazepinones has been investigated. The protocol uses ortho-N-Boc phenylisocyanides and phenylglyoxaldehydes or ethyl glyoxylate in the 4-component Ugi-Azide reaction to afford MCR (Multi Component Reactions) derived adducts equipped with the desired diversity inputs. A subsequent acidic treatment (TFA/DCE) allows a simultaneous deprotection-cyclization leading to the final products.

A peculiar case of bilateral, spontaneous thromboses of the pampiniform plexi.

Acute thrombosis of the pampiniform plexus is an uncommon clinical problem which causes a diagnostic conundrum in men presenting urgently to medical personnel. We present an unusual case of a young man who presented with metachronous thromboses of his pampiniform plexi and review the literature to explore potential aetiologies and therapeutic strategies.
