Article Synopsis
  • Cognitive impairment is common in heart failure (HF) patients, but guidelines for screening and care are insufficiently detailed.
  • A survey of Dutch cardiologists revealed that most do not regularly screen for cognitive impairment, with many expressing the need for more training and support.
  • Implementing a Heart-Brain care pathway could enhance systematic screening and personalized treatment for cognitive impairment in heart failure patients.

Article Synopsis
  • The number of people living with dementia, particularly Alzheimer's disease, is on the rise globally, and they face a heightened risk of cardiovascular diseases, making them potential candidates for hypertension and cholesterol treatments.
  • Current guidelines for cardiovascular risk management (CVRM) do not adequately address the unique needs of dementia patients, as most clinical trials excluded them, creating a gap in effective treatment approaches.
  • A careful evaluation of life expectancy, treatment preferences, and close monitoring for adverse drug effects is crucial for managing hypertension and hyperlipidemia in dementia patients, with an emphasis on adjusting or stopping medications when necessary, especially for those with limited life expectancy.

According to international guidelines, patients with a suspected hypertensive emergency (HE) admitted to the emergency department (ED) should undergo comprehensive evaluation including funduscopic examination. However, funduscopy is not always readily available and little is known about the prevalence of retinopathy among these patients in the ED setting. In order to characterize patients who should undergo funduscopy, we studied the prevalence, characteristics and clinical outcome in patients with a suspected HE and retinopathy grade III/IV.

Background: Hepatitis E virus (HEV) genotype 3 infections can have important clinical consequences.

Objectives: To evaluate patients at risk and the effect of treatment strategies, we studied the clinical course and treatment outcome in patients diagnosed with HEV viremia in our hospital.

Study Design: Between January 2008 and March 2015 we included all patients with HEV genotype 3 (HEV-3) infections diagnosed by means of quantitative real-time reverse transcription-polymerase chain reaction test (RT-PCR).

Brucella is a broad-range, facultative intracellular pathogen that can survive and replicate in an endoplasmic reticulum (ER)-derived replication niche by preventing fusion of its membrane-bound compartment with late endosomes and lysosomes. This vacuolar hijacking was demonstrated to be dependent on the type IV secretion system VirB but no secreted effectors have been identified yet. A virB mutant is unable to reach its ER-derived replicative niche and does not multiply intracellularly.

Both a type IV secretion system and a flagellum have been described in Brucella melitensis. These two multimolecular surface appendages share several features. Their expression in bacteriological medium is growth curve dependent, both are induced intracellularly and are required for full virulence in a mouse model of infection.

Persistence infection is the keystone of the ruminant and human diseases called brucellosis and Malta fever, respectively, and is linked to the intracellular tropism of Brucella spp. While described as non-motile, Brucella spp. have all the genes except the chemotactic system, necessary to assemble a functional flagellum.
