Objectives: There is currently no reliable tool for classifying dementia severity level based on administrative claims data. We aimed to develop a claims-based model to identify patients with severe dementia among a cohort of patients with dementia.
Design: Retrospective cohort study.
Deep Learning models such as Convolutional Neural Networks (CNNs) are very effective at extracting complex image features from medical X-rays. However, the limited interpretability of CNNs has hampered their deployment in medical settings as they failed to gain trust among clinicians. In this work, we propose an interactive framework to allow clinicians to ask what-if questions and intervene in the decisions of a CNN, with the aim of increasing trust in the system.

Recently, contrastive learning based on augmentation invariance and instance discrimination has made great achievements, owing to its excellent ability to learn beneficial representations without any manual annotations. However, the natural similarity among instances conflicts with instance discrimination which treats each instance as a unique individual. In order to explore the natural relationship among instances and integrate it into contrastive learning, we propose a novel approach in this paper, Relationship Alignment (RA for abbreviation), which forces different augmented views of current batch instances to main a consistent relationship with other instances.

Using large-eddy simulation (LES) with virtual walkers implemented, this study investigates pollutant exposure of walking agents (or moving receptors) in an urban turbulent boundary-layer flow developed over an aligned building array under the influence of different wind directions. The spatial variability of the exposure risks are found to be better captured by the moving receptors than the fixed receptors along the same agent walking tracks.

Although pollutant sources are often assumed to be spatially uniform, traffic in real cities may vary significantly in space. Consequently the local air quality within a street may not be determined solely by the traffic volume of the street. Effects of source inhomogeneity on pollutant dispersion from a cubic building array are investigated as a function of the external wind direction. Using building-resolving large-eddy simulation, it is found that the results depend strongly on the source location and source uniformity inside a near-field region defined by a radius of homogenisation (RAD) based on the spatial autocorrelation of the pollutant concentration. The sensitivity of the RAD to the source location changes abruptly around 30° and is greatly reduced for wind angles between 30 and 45°, in agreement with velocity statistics and the mean horizontal streamlines.

The occurrence of trauma as an initiating factor and its early features of pain and inflammation followed by blistering or ulceration mean that it can be mistaken for necrotizing infection. Neutrophilic dermatosis of the dorsal hands should be considered in all patients who present with such features confined to the back of the hands, particularly those with negative microbiological results or lack of response to antibiotic therapy.
