Successful treatments of polyarteritis nodosa cerebral vasculitis and recurrent septicaemia in a dialysis patient.

We report a case of cerebral vasculitis in a 31-year-old woman who presented with chronic kidney disease stage 5, labile hypertension and severe headaches. The diagnosis of cerebral vasculitis made on magnetic resonance angiography (MRA) and late diagnosis of polyarteritis nodosa were made by conventional CT angiography. Immunosuppression was complicated by recurrent septicaemia due to Treatment of the vasculitis resulted in marked improvement of MRA appearances, headaches and anxiety and stabilisation of blood pressure.

Successful treatment of refractory midgut bleeding with ocreotide and corticosteroids in a dialysis patient with suspected sarcoidosis.

We present a case of severe and recurrent small-bowel bleeding, due to multiple intestinal angiodysplasias, in a female patient with chronic renal failure due to suspected sarcoidosis. Over the years, she required numerous admissions and >200 units of blood for symptomatic anaemia. However, following a small-bowel capsule endoscopy that revealed several small-bowel angiectasis, she was treated successfully with octreotide and corticosteroids.

Increased hemodialysis frequency can make fluid overload easier to treat, although most patients are still treated thrice weekly. Online hemodiafiltration (HDF) has recently become an alternative to conventional hemodialysis for treatment of end-stage renal disease with superior results. Clinical experience with HDF in unselected populations has not been widely published and a longitudinal study on such a group of patients is presented here.
