Five cases of twin pregnancy associated with elevated maternal serum alpha-fetoprotein levels are presented. In each instance an abnormality was detected by ultrasound examination--in four cases this represented anomalous development of one or both fetuses: discordant anencephaly; a monozygotic heterokaryotypic twin pregnancy with coexistent Turner's syndrome and intrauterine death; concordant body-stalk syndrome; and fetal exomphalos together with an acardiac monster. In the remaining case, there had been an intrauterine death of one of the twins.

Clearance of microfilariae from the circulation of hamsters infected with Dipetalonema viteae was demonstrated following passive transfer of serum obtained from hyperinfected hamsters. The exclusion fraction after gel filtration of this serum on Sephacryl S 200 also cleared microfilariae whereas the other fractions did not. There was no clearance with serum taken during the pre-patent, patent or latent periods of singly infected hamsters.

As methods for identifying the small-for-dates fetus in twin pregnancies, serial ultrasonic cephalometry and a new two-stage ultrasound examination schedule have been evaluated and compared. Serial cephalometry is associated with technical and interpretative problems, and only 24 (56 per cent) of 43 small-for-dates fetuses in 66 retrospectively studied twin pregnancies showed detectably abnormal biparietal diameter growth. The two-stage schedule comprises an accurate assessment of gestational age in early pregnancy, and determination of the product of the crown-rump length and trunk area at 34 to 36 weeks; this identified all 15 small-for-dates fetuses in 21 prospectively studied twin pregnancies, and the method offers several other advantages over serial cephalometry.

Autonomic mechanisms underlying the initial heart rate response to standing were analyzed in nine normal subjects. The normal pattern of response was altered by atropine to a small and gradual R-R interval shortening over 30 beats, with no rebound R-R interval lengthening. With additional propranolol, R-R interval shortening was even less and confined to the first 15-20 beats, whereas propranolol alone did not affect the normal response pattern, showing that this is under vagal control with increased cardiac sympathetic activity occurring only if the vagus is blocked.

During 7 days of incubation in vitro the trophozoite stage of the free-living soil amoeba, Acanthamoeba polyphaga, phagocytized and killed 78-97% of the cells of three strains of Cryptococcus neoformans. With one strain, incubation time was increased to nine days and 99% of the yeast cells were killed. It was calculated that during 4-9 days of incubation a single trophozoite phagocytized and killed a daily average of 84 yeast cells.

The electrocardiograms in this study of parasystolic rhythm were tape recorded and then analysed with a special purpose hybrid computer. The rate of appearance of specific inter-ectopic intervals was shown to change significantly with small changes in either the sinus or parasystolic pacemaker period. Natural changes in the period of the ectopic pacemakers were observed over several hours.

Native LVG strain hamsters were infected with Dipetalonema viteae by the surgical implantation of adult worms. Groups of hamsters received either 50 male, 50 female, 50 male plus 50 female or 25 male plus 25 female worms per hamster. Approximately 50% of the transferred worms became established in the recipient hosts regardless of the number or sex of the worms implanted.

During experiments on the gastrointestinal tract as a possible portal of entry for Cryptococcus neoformans, we occasionally observed the free-living amoeba, Acanthamoeba polyphaga, growing in the presence of C. neoformans cultured from mouse feces. Examination of the amoebic trophozoites revealed that they were engorged with yeast cells.

Dipetalonema viteae-infected hamsters with amicrofilaremic infections were subjected to immunosuppressive therapy. Methyl prednisolone acetate caused the most severe recrudescence of microfilariae while cyclophosphamide caused a low level, transient microfilaremia. Saline injected control hamsters remained amicrofilaremic.

The immediate heart-rate response to standing was measured in 22 normal controls and 25 patients with diabetes, 15 of whom had autonomic neuropathy. The response in the controls and patients without autonomic neuropathy was characteristic and consistent, with tachycardia maximal at around the 15th beat and relative bradycardia maximal at around the 30th beat. The diabetics with autonomic neuropathy, however, showed a flat response.

Details of a 22-item questionnaire used to assess the result of surgery for peptic ulcer are given. Tested in 47 patients the questionnaire correlates well with the Visick grading. The 100-mm line test and post-operative alkali consumption are less accurate measures of outcome.
