Endoscopic Treatment of Gastroesophageal Reflux Disease.

There have been many devices and ideas to treat reflux disease endoscopically. Several devices have been tried and even FDA approved but now are no longer used. The push for these therapies is to find effective reflux control with lower risk and faster recovery.

In reality, Australia is very far away from addressing the societal level drivers of health inequity. Tackling smoking is an integral component of efforts to improve health outcomes in Aboriginal communities. Social marketing is an effective strategy for promoting healthy attitudes and influencing behaviours; however, there is little evidence for its success in reducing smoking rates in Aboriginal communities. This paper outlines the development, implementation and evaluation of Kick the Habit Phase 2, an innovative tobacco control social marketing campaign in Aboriginal communities in New South Wales (NSW).

Strategies for Aligning Physicians to System Redesign Goals at Eight Safety-Net Systems.

Background: Facing recent economic and regulatory pressures, safety-net systems (SNSs) are redesigning their organizations to improve care delivery, remain financially viable, and maintain competitive positions. Aligning physicians with redesign goals is a priority, particularly as many SNSs shift toward patient-centered, population health-focused models. No previous work has examined efforts to align physicians to safety net redesign efforts.

Overexpression of 14-3-3ζ in cancer cells activates PI3K via binding the p85 regulatory subunit.

The ubiquitously expressed 14-3-3 proteins regulate many pathways involved in transformation. Previously, we found that 14-3-3ζ overexpression increased Akt phosphorylation in human mammary epithelial cells. Here, we investigated the clinical relevance and molecular mechanism of 14-3-3ζ-overexpression-mediated Akt phosphorylation, and its potential impact on breast cancer progression.

Isolated avulsion fracture of the coronoid process requiring open reduction in a paediatric patient: a case report.

Coronoid process fractures are reported to occur from avulsion by the brachialis muscle or to be associated with elbow dislocations. We report a rare case of coronoid process fracture due to avulsion by the anterior bundle of the medial collateral ligament rendering the elbow unstable. In children, small fracture fragments of the coronoid process (types 1 & 2) are in reality often much larger but the actual size is not appreciated radiographically, as the coronoid process contains considerable amounts of cartilage.

High-pressure injection injuries.

A retrospective review of the 11 patients attending the Hand Unit at the Derbyshire Royal Infirmary over the last 5 years with high-pressure injection injuries is presented. The machines and materials that cause these injuries are outlined and the methods of treatment and rehabilitation are described in detail. The study demonstrates the morbidity of high-pressure injection injuries, particularly those inflicted by paint spray guns, and highlights a frequent delay between injury and decompression of the injured part.

Doppler assessment of blood flow to the lower limb associated with total knee arthroplasty.

Arterial blood flow was assessed before and after operation by the pressure index method (Doppler ultrasonography), in 51 consecutive patients treated with primary total knee arthroplasty. This method was chosen since it is non-invasive, inexpensive, easy to apply and readily available in most hospitals. It is a sensitive technique for assessing flow in occlusive arterial disease.

Betamethasone and the human fetal ductus arteriosus.

We used a sensitive Doppler echocardiographic technique to evaluate in utero the effects of betamethasone on the human ductus arteriosus. Transient, mild constriction of the ductus arteriosus 4-5 hours after the first injection of betamethasone occurred in two of 11 trials. The relatively mild and brief effects on the human ductus of betamethasone in usual doses are probably not clinically significant in most instances.

Pathology of the flexor tendon sheath in the spontaneous carpal tunnel syndrome.

The histological features of the flexor tendon sheath in the spontaneous carpal tunnel syndrome were studied. The main differences between our findings and previous studies were twofold. Firstly a striking absence of inflammation in our material and secondly the diversity of the pathological changes encountered--alterations in the connective tissue especially the collagen; proliferation with thickening of the tissues of the tendon sheath; fibrosis; amyloid deposition; oedema; vascular lesions including thickening of vessels walls, intimal hyperplasia, and thrombosis; and a foreign body giant cell reaction.
