A diverse range of relaxation dynamics of trapped spin-states are observed in spin crossover (SCO) materials, including exponential, sigmoidal, stretched exponential, multi-step, and mixed kinetics. We reproduce and explain this full range of relaxation behaviours using a semi-empirical, semi-classical model that combines crystal field theory with elastic inter-molecular interactions. We show that frustrated intermolecular interactions, which are responsible for multistep thermal transitions, also lead to multiple energetically competitive ordered phases, even in systems that contain only one crystallographically distinct SCO site.

Article Synopsis
  • This study examines the link between clinical and imaging features with hormonal subtypes, growth, and surgical treatment of incidental adrenal cortical adenomas (ACAs) in a cohort of patients from 2000 to 2016.
  • Out of 1516 patients, various hormonal subtypes were identified, with factors like age, sex, and tumor size influencing these subtypes, while imaging measures like Hounsfield units (HU) were not significant predictors.
  • The findings indicate younger age, larger tumor size, and increased growth were associated with a higher likelihood of needing adrenalectomy, especially for patients with mild autonomous cortisol secretion (MACS) and nonfunctioning adenomas (NFAs).

Approximately one out of every 88 seniors has inflammatory bowel disease (IBD), and this is expected to increase in the future. They are more likely to have left-sided disease in ulcerative colitis, and isolated colonic disease in Crohn's disease; perianal disease is less common. Other common diagnoses in the elderly must also be considered when they initially present to a healthcare provider.

Discrepancies in phase two and three studies can result in significant patient and financial burden, as well as the nonapproval of potentially efficacious drugs. We aimed to determine whether this discrepancy exists for clinical trials in inflammatory bowel disease (IBD). Electronic databases (MEDLINE and Embase) and clinical trial repositories were searched from 1 January 1946 to 12 March 2021, for paired phase two and three studies of advanced therapies for Crohn's disease and ulcerative colitis.

Background And Aims: While endoscopic retrograde cholangiopancreatography (ERCP) is a valuable diagnostic and therapeutic tool in primary sclerosing cholangitis (PSC), there is conflicting data on associated adverse events. The aims of this systematic review and meta-analysis are to (1) compare ERCP-related adverse events in patients with and without PSC and (2) determine risk factors for ERCP-related adverse events in PSC.

Methods: Embase, PubMed, and CENTRAL were searched between January 1, 2000, and May 12, 2021.

Background: There is increasing interest in direct oral anticoagulants (DOACs), given their safety and convenience in atrial fibrillation, compared with vitamin K antagonists (VKAs). However, the use of DOACs in left ventricular (LV) thrombi is considered off-label, with current guidelines recommending VKAs. The aim of this meta-analysis was to compare the safety and efficacy of DOACs to VKAs in the management of LV thrombi.

Article Synopsis
  • This study investigates how medical students' attitudes towards obesity affect their ability to provide patient-centered care to patients with obesity.
  • Findings indicate that students who believe negative attitudes towards obese patients are common in medical settings exhibit poorer patient-centered behaviors, such as less attentiveness and friendliness.
  • The research suggests that addressing negative attitudes and promoting positive role-modeling in medical education could enhance the quality of care for patients with obesity.

Introduction: Training in high-value, cost-conscious care (HVCCC) is increasingly being incorporated into medical school curricula, but students may have limited opportunities to engage patients in HVCCC conversations. The aim of this study was to develop two standardized patient scenarios with associated checklists, hypothesizing that resulting scores would allow for valid formative assessments of HVCCC communication skills.

Methods: Scenarios were designed to generate a less-is-more conversation (in response to a patient requesting an unnecessary test) and a shared decision-making conversation (in response to a patient choosing between multiple effective treatment options).
