Purpose: Memantine is standard in certain adults receiving brain radiation therapy (RT) to decrease cognitive impacts, but it is unknown whether pediatric patients can take, tolerate, and/or benefit from memantine. In this prospective single-arm feasibility study, we hypothesized that pediatric patients receiving central nervous system (CNS) RT would tolerate memantine with good adherence.

Methods And Materials: Patients aged 4 to 18 years with a primary CNS malignancy (excluding World Health Organization grade 4 astrocytoma, glioblastoma) receiving intracranial RT were eligible.

Article Synopsis
  • Lenvatinib, a multikinase inhibitor, is effective in improving progression-free survival (PFS) in patients with radioiodine-refractory differentiated thyroid cancer, leading to a study on its combination with pembrolizumab.
  • The study enrolled two groups: naïve patients to multikinase inhibitors and those who progressed on lenvatinib, with results showing a 65.5% overall response rate in naïve patients but only 16% in those who previously had lenvatinib.
  • Findings suggest that the combination therapy may improve treatment durability for naïve patients and could serve as a potential salvage option for those whose cancer progressed on lenvatinib alone.

Article Synopsis
  • - The study investigated the effects of the AKT inhibitor MK-2206 on patients with recurrent/metastatic adenoid cystic carcinoma (ACC), a rare and incurable cancer, aiming to reduce MYB expression and induce tumor regression.
  • - Sixteen patients were treated but there were no confirmed responses; however, 13 maintained stable disease, with a median progression-free survival of 9.7 months and overall survival of 18 months.
  • - While MK-2206 did decrease MYB levels in some patients, its clinical efficacy was limited, suggesting a need for new strategies to target MYB in ACC.

Immune checkpoint inhibitors (ICIs), as a novel class of anticancer therapy, can be more efficacious and less toxic than chemotherapy, but their clinical success is confined to certain tumor types. Elucidating their targets, mechanisms and scope of action, and potential synergism with chemotherapy and/or targeted therapies are critical to widen their clinical indications. Treatment response to an ICI targeting programmed death-1 (anti-PD-1) is sought to be understood here by conducting a preplanned correlative analysis of a phase II clinical trial in patients with small bowel adenocarcinoma (SBA).

Guanylyl cyclase C (GUCY2C) is a tumor-suppressing receptor silenced by loss of expression of the luminocrine hormones guanylin and uroguanylin early in colorectal carcinogenesis. This observation suggests oral replacement with a GUCY2C agonist may be an effective targeted chemoprevention agent. Previous studies revealed that linaclotide, an oral GUCY2C agonist formulated for gastric release, did not persist to activate guanylyl cyclase signaling in the distal rectum.

Background: Adipocyte-derived adiponectin may play a role in the host inflammatory response to cancer. We examined the association of plasma adiponectin with the density of tumor-infiltrating lymphocytes (TILs) in colon cancers and with vitamin D, clinicopathological features, and patient survival.

Methods: Plasma adiponectin and 25-hydroxyvitamin D [25(OH)D] were analyzed by radioimmunoassay in 600 patients with stage III colon cancer who received FOLFOX-based adjuvant chemotherapy (NCCTG N0147 [Alliance]).

Article Synopsis
  • Small-bowel adenocarcinoma (SBA) is a rare cancer with no established treatment for advanced cases beyond the initial chemotherapy, and it often shows higher microsatellite instability (MSI-H) which may respond to immune therapy like pembrolizumab.
  • In a study of 40 previously treated patients, pembrolizumab was administered every three weeks, but only 8% showed partial responses, while the majority experienced disease progression and significant adverse effects.
  • The study found that while the overall response didn't meet expectations, a subset of patients with MSI-H tumors showed promising responses, suggesting further investigation in biomarker-specific treatment could be valuable.

Through our involvement in KEYNOTE-059, we unexpectedly observed durable responses in two patients with metastatic gastroesophageal adenocarcinoma (mGEA) who received ramucirumab (anti-VEGFR-2)/paclitaxel after immune checkpoint inhibition (ICI). To assess the reproducibility of this observation, we piloted an approach to administer ramucirumab/paclitaxel after ICI in more patients, and explored changes in the immune microenvironment. Nineteen consecutive patients with mGEA received ICI followed by ramucirumab/paclitaxel.

Background: Oxaliplatin in combination with either 5-fluorouracil or capecitabine is commonly used as first-line therapy for patients with small bowel adenocarcinoma. The addition of irinotecan improves survival in other gastrointestinal tumors but at the cost of hematologic toxicity. The authors performed a phase 2 cooperative group study (North Central Cancer Treatment Group N0543, Alliance) using genotype-dosed capecitabine, irinotecan, and oxaliplatin (gCAPIRINOX), with dosing assigned based on UDP glucuronosyltransferase family 1 member A1 (UGT1A1) genotype to test: 1) whether the addition of irinotecan would improve outcomes; and 2) whether UGT1A1 genotype-based dosing could optimize tolerability.
