Background: ESTIMABL2, a multicentre randomised phase 3 trial in patients with low-risk differentiated thyroid cancer (ie, pT1am or pT1b, N0 [no evidence of regional nodal involvement] or Nx [involvement of regional lymph nodes that cannot be assessed in the absence of neck dissection]), showed the non-inferiority of a follow-up strategy without radioactive iodine (I) administration compared with a postoperative I administration at 3 years post-randomisation. Here, we report a pre-specified analysis after 5 years of follow-up.

Methods: Patients treated with total thyroidectomy with or without prophylactic neck lymph node dissection, without postoperative suspicious findings on neck ultrasonography, were randomly assigned to the no-radioiodine group or to the radioiodine group (1·1 GBq-30 mCi after recombinant human thyrotropin-stimulating hormone).

Article Synopsis
  • The study investigates whether F18-choline (FCH) PET/CT can effectively replace Tc99m-sestaMIBI (MIBI) SPECT/CT for preoperative imaging of parathyroid adenomas in patients with primary hyperparathyroidism (PHPT).
  • It involves a randomized trial across multiple centers in France, enrolling adults eligible for surgery from November 2019 to May 2022, excluding those with prior parathyroid surgery or certain genetic conditions.
  • The primary goal is to compare the success rates of minimally invasive parathyroidectomy guided by these imaging techniques, focusing on patients achieving normal calcium levels one month after surgery.

Article Synopsis
  • Anaplastic thyroid carcinoma (ATC) is a rare and aggressive thyroid cancer, and a study analyzed survival rates using data from 360 patients across France diagnosed between 2010 and 2020.
  • The median overall survival (OS) for all patients was 6.8 months, with better outcomes for those with certain cancer stages and treatments: stage IVb patients had a median OS of 11.4 months, while multimodal treatments significantly improved survival.
  • Key factors that improved OS included better performance status (ECOG 0-1), earlier cancer stage (IVb), and using multimodal treatments, while pure ATC and a high neutrophil-to-lymphocyte ratio (NLR)

Background: In patients with low-risk differentiated thyroid cancer undergoing thyroidectomy, the postoperative administration of radioiodine (iodine-131) is controversial in the absence of demonstrated benefits.

Methods: In this prospective, randomized, phase 3 trial, we assigned patients with low-risk differentiated thyroid cancer who were undergoing thyroidectomy to receive ablation with postoperative administration of radioiodine (1.1 GBq) after injections of recombinant human thyrotropin (radioiodine group) or to receive no postoperative radioiodine (no-radioiodine group).

To evaluate the diagnostic value of FDG PET-CT metabolic parameters and Deauville-like 5 point-scale to predict malignancy in a population of patients presenting focal thyroid incidentaloma (fTI). This retrospective study included 41 fTI, classified according to cytological and histological data as benign (BL) or malignant lesion (ML). FDG PET-CT semi-quantitative parameters (SUVmax, SUVmean, SUVpeak, MTV, TLG), tumor to liver SUVmean ratio (TLRmax and TLRmean), tumor to blood-pool SUVmean ratio (TBRmax and TBRmean) were calculated.

Supradiaphragmatic adrenocorticotropic hormone (ACTH) secreting pituitary adenomas are exceptionally encountered (14 cases previously described) and raise issues concerning their nosology and management. If surgery is the treatment of choice, the nature of surgical approach (craniotomy, transsphenoidal approach) remains controversial. To illustrate this issue, we presented two cases of supradiaphragmatic ACTH secreting pituitary adenomas successfully excised via a subfrontal approach.

Objective: Oestrogens can modulate the action or secretion of GH. Previous studies in postmenopausal women have shown a differential effect between transdermal 17beta-oestradiol and oral ethynyl-oestradiol on GH and IGF-1 concentrations. This secondary analysis, based on a large randomized trial, aimed to estimate the effect of the route of administration of 17beta-oestradiol in combined hormone replacement therapy with progesterone on IGF-1 and IGFBP-3 levels.
