Background: The Brush Sign (BrS) is a radiological biomarker (MRI) showing signal decrease of subependymal and deep medullary veins on paramagnetic-sensitive magnetic resonance sequences. Previous studies have shown controversial results regarding the prognostic value of BrS. We aimed to assess whether BrS on T2*-weighted sequences could predict functional prognosis in patients treated with mechanical thrombectomy (MT).

The introduction of the McDonald criteria has enabled earlier diagnosis of multiple sclerosis (MS). However, even with the 2010 revised criteria, nearly 50% of patients remain classified as "possible MS" following the first MRI. The present study aimed to demonstrate that time to MS diagnosis could be shorter than 2010 revised criteria, and established after a single early MRI in most patients with the association of the symptomatic lesion and at least one suggestive asymptomatic lesion.

Article Synopsis
  • A 26-year-old pregnant woman with a 6-year history of multiple sclerosis (MS) is hospitalized at 17 weeks of pregnancy due to a relapse.
  • MS treatment typically involves high-dose corticosteroids, but a current infection would normally prevent this treatment, raising concerns given her moderately elevated C-reactive protein (CRP) levels (38 mg/L).
  • The literature review shows that while increased CRP can occur during normal pregnancy, there is no agreed-upon standard for interpreting these levels, and procalcitonin testing could help guide treatment decisions despite also being elevated in normal pregnancy.
