In this article, we describe our experience developing and implementing a multipronged approach to improve performance across a strategic subset of quality measures within primary care. Detailed techniques include data visualization and analytics, process reengineering, team engagement, visual project management, continuous improvement methods and training, and incentives and recognition. We achieved positive change across 12 high priority measures which we deemed the "High Value Framework (HVF)" by fostering a collaborative, nonpunitive, problem-solving culture.

Article Synopsis
  • Cytotoxic T lymphocytes (CTLs) are crucial for the immune system as they kill infected cells by receiving signals from antigen presenting cells (APCs) and using lytic granules at the immune synapse (IS).
  • Activation of CTLs involves not just two signals (peptide-MHC and co-stimulation), but a third signal from inflammatory cytokines like interleukin-12 (IL-12) that enhances their function.
  • Recent studies using live cell microscopy revealed that CTLs activated by three signals exhibit distinct cytoskeletal dynamics and generate greater mechanical forces compared to those activated by two signals, affecting their ability to kill target cells.

Activation of T-cells leads to the formation of immune synapses (ISs) with antigen-presenting cells. This requires T-cell polarization and coordination between the actomyosin and microtubule cytoskeletons. The interactions between these two cytoskeletal components during T-cell activation are not well understood.
