Pulmonary embolism is a significant cause of morbidity and mortality in pregnancy and the puerperium. In severe cases, it causes haemodynamic instability and can lead to cardiac arrest due to obstructive shock. Patients with acute PE can be risk-stratified, to guide their monitoring and treatment; this article focuses on intermediate and high-risk PE.

Association of Adenoid Hypertrophy with Nasal and Middle Ear Pathologies in Adults.

A prospective comparative study was carried out in patients attending ENT Out Patient Department of a tertiary care Hospital with an aim to determine the association of adenoid hypertrophy with chronic nasal and middle ear pathologies in those aged 18 years or above. Patients presenting between August 2020 and February 2022 with chronic nasal and middle ear pathologies who underwent diagnostic nasal endoscopy were seen for adenoid hypertrophy and its association was analysed statistically. Adenoid hypertrophy (AH) in 18-25, 26-35, 36-45 and 46-55 years age group are 56.

Potential Lifshitz transition at optimal substitution in nematic pnictide BaSrNiAs.

BaNiAs is a structural analog of the pnictide superconductor BaFeAs, which, like the iron-based superconductors, hosts a variety of ordered phases including charge density waves (CDWs), electronic nematicity, and superconductivity. Upon isovalent Sr substitution on the Ba site, the charge and nematic orders are suppressed, followed by a sixfold enhancement of the superconducting transition temperature (). To understand the mechanisms responsible for enhancement of , we present high-resolution angle-resolved photoemission spectroscopy (ARPES) measurements of the BaSrNiAs series, which agree well with our density functional theory (DFT) calculations throughout the substitution range.

Fecal Sludge Treatment or Septage Management is increasingly being recognised as an effective and appropriate method to scale urban sanitation systems to achieve safe sanitation, particularly in small towns and cities. As implementation progresses, data-based evidence is emerging, highlighting the challenges faced on the ground, and the requisite planning necessary to address them. This paper presents the findings, challenges and possible ways ahead from a study conducted to provide data for Fecal Sludge Management (FSM) planning for a small town in a state in southern India.

A van der Waals antiferromagnetic topological insulator with weak interlayer magnetic coupling.

Magnetic topological insulators (TI) provide an important material platform to explore quantum phenomena such as quantized anomalous Hall effect and Majorana modes, etc. Their successful material realization is thus essential for our fundamental understanding and potential technical revolutions. By realizing a bulk van der Waals material MnBiTe with alternating septuple [MnBiTe] and quintuple [BiTe] layers, we show that it is ferromagnetic in plane but antiferromagnetic along the c axis with an out-of-plane saturation field of ~0.

Background: The Pde6brd1 (Rd1) mouse is widely used as a murine model for human retinitis pigmentosa. Understanding the spatio-temporal patterns of cone degeneration is important for evaluating potential treatments. In the present study we performed a systematic characterization of the spatio-temporal patterns of S- and M/L-opsin cone outer segment and cell body degeneration in Rd1 mice, described the distribution and proportion of dual cones in Rd1 retinas, and examined the kinetics of microglial activation during the period of cone degeneration.

A novel anti-melanoma SRC-family kinase inhibitor.

The major drawback of melanoma therapy with BRAF and MAPK inhibitors is the innate and acquired drug resistance. We therefore explored alternative targets and developed a new compound, SAB298, that is a SRC-family kinase (SFK) inhibitor. The drug is cytotoxic to patient-derived melanoma cells regardless of oncogene expression and inhibits tumor growth .

Altered In Vivo Lipid Fluxes and Cell Dynamics in Subcutaneous Adipose Tissues Are Associated With the Unfavorable Pattern of Fat Distribution in Obese Adolescent Girls.

Patterns of abdominal fat distribution (for example, a high vs. low visceral adipose tissue [VAT]/[VAT + subcutaneous adipose tissue (SAT)] ratio), independent of obesity, during adolescence carry a high risk for insulin resistance and type 2 diabetes. Longitudinal follow-up of a cohort of obese adolescents has recently revealed that a high ratio (high VAT/[VAT + SAT]) is a major determinant of fatty liver and metabolic impairment over time, with these effects being more pronounced in girls than in boys.

An Algorithm for the Management of Residual Head and Neck Melanoma In Situ Using Topical Imiquimod: A Pilot Study.

Introduction: Melanoma in situ (MIS) is an early variant of melanoma in which the disease is limited to the epidermis. Standard therapy is currently surgical excision with at least 5-mm margins. In some instances, there are large or anatomically difficult lesions where complete excision will result in significant esthetic or functional impairment.

Introduction: Melanoma is the deadliest form of skin cancer and has the ability to metastasize widely. Accurate diagnosis of sentinel lymph nodes (SLN) is crucial to its management. The gold standard for SLN identification is planar lymphoscintigraphy.

Management of intussusception in patients with melanoma.

Background: Increased cross-sectional imaging for surveillance of metastatic melanoma has led to more diagnoses of asymptomatic intussusception.

Methods: We performed a multi-institutional retrospective review of patient records with a history of metastatic melanoma and a diagnosis of intussusception. Patients were divided into three groups: 1) asymptomatic patients without current evidence of melanoma (no evidence of disease [NED]); 2) asymptomatic intussusception and known active metastatic melanoma; 3) symptomatic intussusception and known active metastatic melanoma; the number of patients requiring surgery and intraoperative findings were recorded.

Coil Embolisation of Post Traumatic Giant Supraclinoid Pseudoaneurysm Presenting as Carotid Cavernous Fistula.

This report documents the occurrence, diagnosis and treatment of a 29 year-old male patient who presented with progressive swelling of the left eye with associated progressive loss of vision, three months after sustaining a closed head injury in a road traffic accident. Magnetic resonance imaging (MRI) showed a large, ill-defined lesion in the left supraclinoid and paraclinoid region with variable contrast enhancement. A four vessel Digital Subtraction Angiography (DSA) showed a large, supra-clinoid pseudoaneurysm which had ruptured inferiorly into the roof of cavernous sinus, forming direct carotid cavernous fistula (CCF) which lead to dilatation of the superior ophthalmic vein and subsequent peri-orbital oedema and chemosis of conjunctiva.

Pain Management and Body Contouring.

Body contouring procedure is a catchall that includes mastopexy, abdominoplasty, liposuction, and lower body lift. It is well known that optimal postoperative pain control decreases hospitalization length of stay and promotes early ambulation, which in turn decreases the incidence of complications. There have been many innovations in pain control beyond opiate medications, including prolonged administration of local anesthesia, regional blocks, and the use of a multimodal regimen which utilizes nonopiates.

Nonvisualization of Sentinel Lymph Nodes by Lymphoscintigraphy in Primary Cutaneous Melanoma: Incidence, Risk Factors, and a Review of Management Options.

Background: Lymphoscintigraphy is often performed before sentinel lymph node biopsy, especially in areas likely to have multiple or aberrant drainage patterns. This study aims to determine the incidence and characteristics of melanoma patients with negative lymphoscintigraphic findings and to review the management options and surgical recommendations.

Methods: This is a retrospective study of patients with primary cutaneous melanoma who underwent sentinel lymph node biopsy between 2005 and 2016.

Eliminating Preoperative Lymphoscintigraphy in Extremity Melanomas.

Background: Preoperative lymphoscintigraphy (LSG) is an imaging procedure routinely used to identify the draining nodal basin in melanomas. At our institute, we have traditionally performed preoperative LSG followed by intraoperative LSG for logistical and evaluative reasons. We sought to determine if preoperative LSG could be safely eliminated in the treatment of extremity melanomas, which exhibit consistent and predictable lymphatic drainage patterns.
