Hand amputation is a devastating, usually accidental, injury. -implantation of the severed hand is a challenging procedure that requires careful planning, precise microsurgical techniques, and adequate vascular supply. Successful surgical reimplantation therefore requires foresight into unique variant neurovascular anatomy that could be encountered and how these would pose challenges during the procedure.

Introduction/importance: Crush injury with extensive soft tissue damage to an extremity is generally considered a contraindication to re-implantation and the extremity best managed by prosthesis. However good prosthesis is not easily available especially in resource constrained environment, the overall long term quality of life is also considered better with re-implantation.

Reconstruction of mid- and forefoot defects involves few options as versatile as free flaps; however, free flaps are not universally available and require long operative time and can be difficult to successfully heal. Although the reverse sural flap is commonly used for lower limb reconstructions, it has been used mainly for reconstruction of defects around the distal leg, ankle, and hindfoot. Few studies have demonstrated the use of this flap in the reconstruction of mid- and forefoot defects.

Persistent posttraumatic CSF fluid leakage may present a challenge to manage. Failure to address the leakage may result in complications such as meningitis, septicemia, radiculopathy, muscle weakness, and back pains. While the majority of the leakages may be managed conservatively, large dura defects as a result of gunshot wounds or motor vehicle accidents are best managed by surgical interventions.
