Echocardiographic findings in the fastest growing segment of our population, the very elderly, are limited in the literature. We performed a retrospective analysis of 431 consecutive nonagenarians who underwent transthoracic echocardiography (2-dimensional, M-mode, pulse and continuous wave Doppler with color flow mapping) at our center. In this report, the case of a 40-year-old patient with chronic total occlusion of the left main coronary artery is discussed. Total occlusion of the left main coronary artery is a rare finding at coronary angiography. When present, patients most often have extensive collateral circulation from the right coronary artery.
