Article Synopsis
  • * A study looked at patients with sepsis and divided them into two groups: those with kidney problems (AKI) and those without.
  • * After a blood treatment called cPED, APN levels went up in both groups, suggesting it could be helpful for people with sepsis.

  • The study aimed to examine bear-related trauma in Japan, focusing on injury patterns, outcomes, and treatment approaches.
  • Conducted in 2023 at Akita University Hospital, it analyzed medical records of 20 patients, revealing most injuries occurred in human habitats, primarily affecting the face.
  • Results showed that the majority of patients required emergency interventions, such as surgery and ICU care, with a notable percentage experiencing significant long-term effects, emphasizing the need for better preventive strategies and treatment protocols.

Background: Direct reinfusion of pericardial blood during cardiac surgery triggers a systemic inflammatory response. Although various inflammatory mediators have been identified as triggers, the role of damage-associated molecular patterns (DAMPs) remains poorly understood. Despite guidelines recommending against this practice owing to its harmful effects, it is sometimes used in emergencies.

Article Synopsis
  • Early intervention through mechanical thrombectomy (MT) for patients with large vessel occlusion (LVO) can improve outcomes, but effective prehospital detection and transport strategies were lacking before this study.
  • The study analyzed the implementation of an emergent large vessel occlusion (ELVO) screening system by emergency medical services (EMS) in Akita, Japan, and its effects on patient transport times to MT-capable hospitals.
  • Results showed that the triage group experienced shorter transport times with a notable 14-minute reduction in one hospital, and the ELVO screen had a sensitivity of 87% for accurately identifying LVO cases.

Ascites is the accumulation of fluid in the abdominal cavity and is commonly attributed to various etiologies, including portal hypertension and peritoneal diseases. Hyperthyroidism is rarely associated with ascites, which is typically chylous and accompanied by high central venous pressure. We present a unique case of a 57-year-old woman with untreated hyperthyroidism who manifested non-chylous ascites without evidence of high venous pressure.

Introduction: Coronavirus disease 2019 (COVID-19) is associated with a marked increase in the inflammatory cytokines, IL-6 and IL-18. Blood purification therapy aimed at controlling cytokines is one treatment option; however, evidence of its effectiveness is needed. Plasma exchange with dialysis (PED) is a blood purification method involving selective plasma exchange with dialysate flowing through the outer hollow fiber of the plasma separator.

Article Synopsis
  • A randomized controlled trial tested the effects of Kampo medicine (kakkonto and shosaikotokakikyosekko) combined with standard treatment for COVID-19 symptom relief.
  • The post hoc analysis revealed that symptoms disappeared faster in the Kampo group compared to the control group, though these results weren't statistically significant overall.
  • Specific symptoms like fever and shortness of breath showed significantly faster disappearance with Kampo treatment, particularly in unvaccinated patients, highlighting its potential benefits for acute COVID-19.

Background: Kampo prescriptions can cause drug-induced lung injury (DLI) and acute respiratory distress syndrome (ARDS). However, severe respiratory failure induced by Otsujito (OJT) is extremely rare. High-dose steroid pulse therapy is generally given to patients with severe DLI.

Chest pain is an important symptom for emergency physicians. It is one of the most common causes for admission in emergency departments. Acute leukemia (AL) rarely causes chest symptoms due to ostalgia, and it is difficult to diagnose leukemia as the cause of chest pain.

Background: In Kampo medicine, tongue examination is used to diagnose the pathological condition "Sho," but an objective evaluation method for its diagnostic ability has not been established. We constructed a tongue diagnosis electronic learning and evaluation system based on a standardized tongue image database.

Purpose: This study aims to verify the practicality of this assessment system by evaluating the tongue diagnosis ability of Kampo specialists (KSs), medical professionals, and students.

Drug-induced liver injury (DILI) is difficult to diagnose as it presents with a wide variety of clinical manifestations and there is no established specific biomarker. However, clinicians require expertise in diagnosing DILI as it can lead to critical illness, is relatively common, and can be caused by a variety of drugs, herbal medicines, and supplements. A 67-year-old male was admitted to the hospital with a fever, jaundice, and fatigue.

The traditional Japanese (Kampo) medicine, kakkonto with shosaikotokakikyosekko, has antiviral and anti-inflammatory effects. In this randomized trial, patients with mild and moderate coronavirus disease (COVID-19) were randomly allocated to the control group receiving conventional treatment for symptom relief such as antipyretics and antitussives or the Kampo group receiving mixed extract granules of kakkonto (2.5 g) and shosaikotokakikyosekko (2.

Objective Patients in whom coronavirus disease 2019 (COVID-19) was suspected or confirmed between January 1, 2020, and October 31, 2021, were enrolled from Japanese hospitals in this multicenter, retrospective, observational study. Methods Data on the treatment administered (including conventional and Kampo medicine) and changes in common cold-like symptoms (such as fever, cough, sputum, dyspnea, fatigue, and diarrhea) were collected from their medical records. The primary outcome was the number of days without a fever (with a body temperature <37°C).

Tongue examination is an important diagnostic method for judging pathological conditions in Kampo (traditional Japanese medicine), but it is not easy for beginners to learn the diagnostic technique. One reason is that there are few objective diagnostic criteria for tongue examination findings, and the educational method for tongue examination is not standardized in Japan, warranting the need for a tongue image database for e-learning systems that could dramatically improve the efficiency of education. Therefore, we constructed a database comprising tongue images whose findings were determined on the basis of votes given by five Kampo medicine specialists (KMSs) and confirmed the educational usefulness of the database for tongue diagnosis e-learning systems.
