Findings on serial thigh MRI can identify individuals with inflammatory myositis who recover sub-optimally: A 6-month follow-up study.

Background: Magnetic resonance imaging (MRI) of the thigh is used in diagnosis of idiopathic inflammatory myositis (IIM) diagnosis due to its high sensitivity in detecting muscle oedema and to localize the site for muscle biopsy. At the same time, dual energy absorptiometry (DXA) is an accepted method in clinical practice to measure muscle mass and change in body composition. In this longitudinal study of patients with active IIM we sought to correlate muscle findings on serial thigh MRI and body composition assessed using DXA with six-month clinical outcomes, we also studied correlation of thigh MRI scores with body composition parameters.

Implementation of pharmacogenomics: Experience at Ochsner Health.

Purpose: Pharmacogenomics is the study of how individual responses to medication are influenced by genetics. As medication experts, pharmacists are uniquely suited to practicing this application of precision medicine. Fundamental operational aspects of developing a pharmacy clinical pharmacogenomics practice are described, and the specific experience of a multisite health system in developing a pharmacogenomics service is discussed.

Identification of stable restorers and high-yielding hybrids using diverse sorghum male sterile cytoplasmic sources and established pollen parents under different water regimes.

Sorghum hybrids demonstrated increased productivity and helped offset the decreasing cultivated areas, particularly in Asia. The diversity in cytoplasmic male sterility systems, stability of restorers and high yield of sorghum is an important factor for achieving food security and sustainability. In sorghum, hybrid production has been limited to A1 cytoplasmic source to date, primarily due to limited number of restorers on other cytoplasmic sources.

Meckel's diverticulum is a common congenital anomaly of the gastrointestinal (GI) system, which remains asymptomatic unless some complications occur. Littre's hernia is a rare complication of Meckel's diverticulum, where it herniates into the hernial sac. It is difficult to diagnose this condition preoperatively.

Adverse alterations of the microbiota or dysbiosis can lead to chronic inflammation.

Inguinal Lipoblastoma Masquerading as Obstructed Omentocele in a Girl.

Lipoblastoma in the inguinal regional is a rare occurrence in children and can present as a surgical surprise during pediatric herniotomies. Irreducible inguinal hernia is one of the most common surgical problems dealt by pediatric surgery residents in emergency. We report a case of inguinal lipoblastoma presenting as irreducible groin swelling.

Designing a productive, profitable integrated farming system model with low water footprints for small and marginal farmers of Telangana.

Background: Atrial fibrillation and atrial flutter represent the most prevalent clinically significant cardiac arrhythmias. While the CHA2DS2-VASc score is commonly used to inform anticoagulation therapy decisions for patients with these conditions, its predictive power is limited. Therefore, we sought to improve risk prediction for left atrial appendage thrombus (LAAT), a known risk factor for stroke in these patients.

Effect of Combined Intervention of Yoga and Physiotherapy as Compared to Only Physiotherapy on Lower Limb Muscle Strength, Balance and Range of Motion among Elders with Compromised Stair Climbing in Puducherry - A Parallel Arm Non-Randomized Control Trial.

The activity of daily living (ADL) related to stair climbing is one of the most compromised ADLs among the elders. It requires good muscle strength, balance, and range of motion (ROM) in the lower limb. We aimed to investigate the effects of Yoga complemented with physiotherapy (Intervention group) compared to only physiotherapy (control group) on lower limb muscle strength, balance, and ROMs among elders with ADL limitation in stair climbing.

Effects of Water Sorption and Solubility on Degradation of Silorane and Methacrylate-Based Dental Composites.

Introduction: Wet oral environment may have deleterious effects on performance of the composites due to influences of water sorption and solubility. The study evaluated the hydrolytic degradation caused because of water sorption and solubility of silorane and methacrylate-based dental composites.

Methods: Ten disc samples (2 mm × 10 mm) were prepared.

Expanding treatment horizons: transcatheter mitral valve-in-valve replacement in bridging to heart transplant.

Transcatheter mitral valve (MV) replacement is an option in complex MV disease with potentially more predictable treatment outcomes and less invasiveness that increases the treatment spectrum to high-risk or non-surgical patients. We present an interesting case of failed MV bioprosthesis who presented with progressively worsening heart failure for heart transplant evaluation, but was successfully managed with transcatheter mitral valve-in-valve replacement (TMViVR). This case illustrates the pivotal role of TMViVR utilization in addressing complex cardiac scenarios, particularly when re-operative surgical MV replacement poses a high risk and also bridging the gap between the present and future heart transplantation preparations by paving the way for a well-prepared patient in the subsequent phase.

Clinical, Hormonal, and Genetic Spectrum of 46 XY Disorders of Sexual Development (DSD) Patients.

Objectives: To evaluate the clinical, hormonal and genetic characteristics of 46XY disorders of sexual development (DSD) patients from South India.

Methods: 46XY DSD patients with a provisional diagnosis of 17β-hydroxysteroid dehydrogenase 3 (17BHSD3) deficiency, 5 alpha-reductase type 2 deficiency (5ARD2) or partial androgen insensitivity syndrome (PAIS) based on clinical and hormonal analysis were included in this study. All the patients underwent detailed clinical and hormonal evaluations.

Congenital Intraoral Dermoid and Epidermoid Cyst with Orocutaneous Fistula Presenting with Life-threatening Airway Obstruction in an Infant.

Dermoid cysts and epidermoid cysts in the floor of the mouth are rare in the pediatric age group. In this case report, we have discussed the presentation, management, and literature review of a 6-month-old female child presenting with both dermoid and epidermoid cysts in the floor of the mouth with an orocutaneous fistula.

MicroRNA-mediated epigenetic regulation of HDAC8 and HDAC6: Functional significance in cervical cancer.

Cervical cancer, a leading global cause of female mortality, exhibits diverse molecular aberrations influencing gene expression and signaling pathways. Epigenetic factors, including histone deacetylases (HDACs) such as HDAC8 and HDAC6, along with microRNAs (miRNAs), play pivotal roles in cervical cancer progression. Recent investigations have unveiled miRNAs as potential regulators of HDACs, offering a promising therapeutic avenue.

Determining the Best Thyroid Imaging Reporting and Data System: A Prospective Study Comparing the Diagnostic Performance of ACR, EU, and K TIRADS in the Evaluation of Thyroid Nodules.

 Many different risk stratification systems have been formulated for thyroid nodules, differing in their fine-needle aspiration cytology (FNAC) indication, suggesting a lack of consensus around the world.  This prospective study was conducted to find the best guideline for risk stratification, for a better malignancy yield, and with reduced rates of negative FNACs among three Thyroid Imaging, Reporting, and Data System (TIRADS) guidelines.  A total of 625 thyroid nodules with conclusive FNAC or histopathological diagnosis were included in the study.

Insulin Sensitivity, Islet Cell Function, and Incretin Axis in Pregnant Women With and Without Gestational Diabetes Mellitus.

Introduction: The aim of this study was to compare insulin sensitivity, islet cell function, and incretin axes in pregnant subjects with GDM and normal healthy controls.

Methods: Pregnant women at 24 to 28 weeks of gestation were subjected to a 75 g oral glucose tolerance test (OGTT). Samples for glucose, insulin, glucagon, glucagon-like peptide-1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) were collected at 0, 30, 60, and 120 min during the OGTT.

Surgery for a symptomatic massive liver haemangioma.

Liver haemangiomas are the most common benign hepatic tumours, but secondary portal hypertension resulting from haemangiomas is exceedingly uncommon. We present a case of a man in his 50s who presented with a progressively enlarging mass in the right upper abdomen. CT of the liver revealed a large hypodense lesion involving the right lobe, with two smaller lesions in the left lobe.

Late Presentation of Right Coronary Artery Stent Infection as Left Empyema and Pleuropericardial Fistula.

Coronary stent infection (CSI) is a rare but potentially life-threatening disease, the incidence of which has been on a rise over the past 2 decades. Surgery seems the treatment of choice, providing a definitive diagnosis of CSI and removing the source of infection, repairing aneurysms, and providing bypass vascular grafts if feasible. Thorough evaluation and a multidisciplinary approach lead to detection and a better outcome.

Sediment-associated microplastics in Chilika lake, India: Highlighting their prevalence, polymer types, possible sources, and ecological risks.

The primary objective of this research was to assess microplastics (MPs) in the sediments of Chilika lake. MPs were extracted from 22 sediment samples using the density separation method combined with vacuum pump filtration. A stereo-zoom microscope and Raman spectroscopy were employed to identify the sediment-associated MPs.

A Real Headache: Intracranial Extension and Epidural Abscess As Complication of Chronic Mucocele.

Mucoceles are benign lesions of salivary glands typically originating from the paranasal sinuses. Intracranial extension and superinfection of these lesions are rare but serious complications of chronic mucoceles. Here, we discuss a patient with a known mucocele, initially lost to follow-up, who presented three years later with headache, purulent rhinorrhea, and intracranial extension of his mucocele with development of an epidural abscess.

Genetic counseling for pre-implantation genetic testing of monogenic disorders (PGT-M).

Pre-implantation genetic testing (PGT) is a vital tool in preventing chromosomal aneuploidies and other genetic disorders including those that are monogenic in origin. It is performed on embryos created by intracytoplasmic sperm injection (ICSI). Genetic counseling in the area of assisted reproductive technology (ART) has also evolved along with PGT and is considered an essential and integral part of Reproductive Medicine.
