Importance: Airway stenosis is a rare but debilitating disorder that significantly degrades the quality of life in affected patients. Treatments are primarily surgical, and disease management lacks established medical therapies. The North American Airway Collaborative held its third symposium at The Johns Hopkins Hospital in Baltimore, Maryland, on April 15, 2024, focused on strategies to advance the care of these patients.

Modeling maize aflatoxins and fumonisins in a Tanzanian smallholder system: Accounting for diverse risk factors improves mycotoxin models.

Human exposure to mycotoxins is common and often severe in underregulated maize-based food systems. This study explored how monitoring of these systems could help to identify when and where outbreaks occur and inform potential mitigation efforts. Within a maize smallholder system in Kongwa District, Tanzania, we performed two food surveys of mycotoxin contamination at local grain mills, documenting high levels of aflatoxins and fumonisins in maize destined for human consumption.

Profiling of insulin-resistant kidney models and human biopsies reveals common and cell-type-specific mechanisms underpinning Diabetic Kidney Disease.

Diabetic kidney disease (DKD) is the leading cause of end stage kidney failure worldwide, of which cellular insulin resistance is a major driver. Here, we study key human kidney cell types implicated in DKD (podocytes, glomerular endothelial, mesangial and proximal tubular cells) in insulin sensitive and resistant conditions, and perform simultaneous transcriptomics and proteomics for integrated analysis. Our data is further compared with bulk- and single-cell transcriptomic kidney biopsy data from early- and advanced-stage DKD patient cohorts.

Very low sulfur fuel oil spilled from the MV Wakashio in 2020 remains in sediments in a Mauritius mangrove ecosystem nearly three years after the grounding.

The oil spill resulting from the grounding of the MV Wakashio on a reef off the coast of Mauritius in July 2020 was the world's first major spillage of Very Low Sulfur Fuel Oil (VLSFO) since the implementation of a Global Sulfur Cap from January 2020. In this study, we examine sediments collected in March 2023 from two Mauritius mangrove systems. Analyses by both gas chromatography-mass spectrometry and comprehensive two-dimensional gas chromatography confirmed, by comparison of molecular biomarkers, the presence of Wakashio VLSFO in one of the mangrove systems.

Feasibility of Using Inertial Measurement Units (IMUs) to Augment Cadaveric Temporal Training.

Objective: Insertional speed of cochlear implant electrode arrays (EA) during surgery is correlated with force. Low insertional speed, and therefore force, may allow for preservation of intracochlear structures leading to improved outcomes. Given the importance of low insertional speeds, we investigate the feasibility of using inertial sensors for kinematic analysis during EA insertion to augment otolaryngology-head and neck surgery training.

Cost-Effectiveness of Temporary Financial Assistance for Veterans Experiencing Housing Instability.

Importance: The US Department of Veterans Affairs (VA) partners with community organizations (grantees) across the US to provide temporary financial assistance (TFA) to vulnerable veterans through the Supportive Services for Veteran Families (SSVF) program. The goal of TFA for housing-related expenses is to prevent homelessness or to quickly house those who have become homeless.

Objective: To assess the cost-effectiveness of the SSVF program with TFA vs without TFA as an intervention for veterans who are experiencing housing insecurity.

Classification of Predictors of Rapid Development of Kidney Failure and Short-Term Changes in Concentration of Circulating Proteins.

Objective: Limited knowledge exists regarding short-term changes/increases in concentrations of circulating proteins (referred here as deltas) and rapid development of kidney failure (rapid KF) in diabetes mellitus.

Research Design And Methods: Concentrations of 452 circulating proteins were measured by OLINK proteomics platform at baseline and after a median interval of 3-4 years in 106 individuals with type 1 and 77 with type 2 diabetes in two case-control studies. During 10-year follow-up, 31 and 26 individuals, respectively, developed rapid KF.

Objective: Learning Health Systems (LHSs) have not directly informed evidence-based policymaking. The Translation-to-Policy (T2P) Learning Cycle aligns scientists, end-users, and policymakers to support a repeatable roadmap of innovation and quality improvement to optimize effective policies toward a common public health goal. We describe T2P learning cycle components and provide examples of their application.
