Background: The world population is aging, with octogenarians expected to reach over 400 million by 2050. Acute cholecystitis is a serious complication in the elderly. Age is not a contraindication for emergency cholecystectomy, an option that can both save lives and preserve quality of life.

Background/purpose: There is uncertainty about the role of prophylactic intra-abdominal drains after distal pancreatectomy. In the present study, we aimed to describe the long-term outcomes of postoperative pancreatic collections in patients who underwent a minimally invasive distal pancreatectomy (MIDP) without surgical drain placement.

Methods: From 2018 to 2022, consecutive patients who underwent a MIDP were recorded.

Background: Intraductal papillary neoplasm of the bile ducts (IPNB) is a rare disease in Western countries. The aim of this study was to compare tumor characteristics, management strategies, and outcomes between Western and Eastern patients who underwent surgical resection for IPNB.

Methods: A multi-institutional retrospective series of patients with IPNB undergoing surgery between January 2010 and December 2020 was gathered under the auspices of the European-African Hepato-Pancreato-Biliary Association (E-AHPBA), and at Nagoya University Hospital, Japan.

Article Synopsis
  • Lymph-nodal involvement (N+) is a negative prognostic factor for patients undergoing pancreatoduodenectomy for pancreatic adenocarcinoma, and existing preoperative diagnostic tools are not very effective at detecting it.
  • This study analyzed data from 2034 patients to determine if preoperative levels of the tumor marker CA19.9 could predict N+ status in resectable pancreatic cancer (R-PDAC).
  • The results indicated a significant association between elevated CA19.9 levels (above 37 U/L) and N+ status, particularly in patients classified as cT3, suggesting that high CA19.9 could help identify patients at greater risk for lymph-nodal involvement before surgery.

Background: Lymphatic spread of intrahepatic cholangiocarcinoma (iCCA) is common and negatively impacts survival. However, the precise role of lymph node dissection (LND) in oncologic outcomes for patients with intrahepatic cholangiocarcinoma remains to be established.

Methods: Updated evidence on the preoperative diagnosis and prognostic value of lymph node metastasis is reviewed, as well as the potential benefit of LND in patients with iCCA.
