Background: Traction is used at our hospital before open reduction in infants with developmental dysplasia of the hip. Theoretically, it reduces soft-tissue tension, allowing an easier surgical reduction and therefore lower surgical complications. Owing to extended hospital stays, potential complications, and lack of evidence, the use of traction has decreased.

Introduction: Neck of femur fractures (NOF) in younger adults are often treated by head-preserving fixation procedures. Non-union is an infrequent but troublesome complication following internal fixation, especially in atypical presentations. Vascularized (VFG) and non-VFG (NVFG) fibular grafting and realignment procedures have been described as an adjunct to internal fixation.

Background: The proximal femoral nail is a commonly used fixation device for extra-capsular neck of femur fractures at our UK NHS Trust. Fracture-related infection (FRI) is a catastrophic complication that can be associated with internal fixation. FRI is often diagnosed late, and causes significant impact on the patient and healthcare system, leading to extended hospital stays, reduced quality of life, high healthcare costs and increased mortality and morbidity.

Article Synopsis
  • - The study compared costs associated with two strategies for assessing stable chest pain: the experimental approach using CT coronary angiography (CTCA) with fractional flow reserve (FFR) versus standard care paths, revealing no significant cost differences in the US healthcare system.
  • - Among the 1,399 randomized patients, different initial tests were planned, with costs slightly higher for the experimental strategy; however, costs were lower in cases involving invasive angiography.
  • - Key factors influencing costs included age, sex, pre-existing conditions, and post-randomization procedures, highlighting that coronary risk profiles significantly impacted cardiovascular care expenses.
