Left ventricular assist device "Incor" ("Berlinhear", Germany) implantation experience in patient with ischemic cardiomiopathy and severe congestive heart failure is presented. Left ventricular assist device implantation was followed by coronary artery bypass grafting simultaneously. Total assist time was 211 days.

The object of the work was to study the effectiveness of surgical (using extracorporeal circulation and on the "functioning heart") and endovascular (using stents) methods for treatment of elderly and old patients suffering from coronary heart disease (CHD) and multi-vascular affection of coronary arteries (CA). Endovascular operations with the use of stents with drug covering considerably increase the duration of normal functioning of the stented CA part in elderly and old patients coupled with decreased traumatization and proliferation of the vascular intima. Modern technology of fulfillment and gained experience make it possible to decrease considerably the invasiveness and to enhance angioplasty safety.

In order to assess the effect of coronary shunting (CS) on the diastolic function of the left ventricle (DFLV) in sick people of elderly and old age with ischemic heart disease stress-Doppler-echocardiogram was performed on 45 men at the age of 65 to 75 years before and after CS. The patients were formed into two groups on the basis of preoperative study of DFLV. Group I consisted of patients who had no myocardial infarction (MI) or who have had small-focal MI, parameters of transmitral blood flow in rest corresponded to impaired diastolic function on the "disturbed relaxation" type.

Last years the progress in development and improvement of coronary surgery and angioplasty, with shunting with patients of elderly and senile age, has been observed in the Russian Federation. This is connected, in the first place, with the progress in timely diagnostics of coronary artery disease, development of surgical equipment, anaesthesiology and resuscitation, introduction of new methods of intra-operative protection of myocardium, development and active use of modern methods of treatment of patients before and after the operation. However despite the versatility of existing scientific schools and literary sources covering this problem, the discussions on indications and contraindications to active surgical treatment of coronary artery disease with the patients of the given age group still continue.
