Background: Many gastric cancer patients in Western countries are diagnosed as metastatic with a median overall survival of less than twelve months using standard chemotherapy. Innovative treatments, like targeted therapy or immunotherapy, have recently proved to ameliorate prognosis, but a general agreement on managing oligometastatic disease has yet to be achieved. An international multi-disciplinary workshop was held in Bertinoro, Italy, in November 2022 to verify whether achieving a consensus on at least some topics was possible.

Hormone receptor (HR)-positive/HER2-positive breast cancer represents a distinct subtype expressing estrogen and progesterone receptors with an overexpression of HER2. Approximately 14% of female breast cancer cases are HER2-positive, with the majority being HR-positive. These tumors show a cross-talk between the hormonal and HER2 pathways; the interaction has implications for the treatment options for the disease.

Muscular dystrophies (MDs) are incurable genetic myopathies characterized by progressive degeneration of skeletal muscles. Dystrophic mice lacking the transcription factor Nfix display morphological and functional improvements of the disease. Recently, we demonstrated that MAPK signaling pathway positively regulates Nfix in muscle development and that Cyanidin, a natural antioxidant molecule, strongly ameliorates the pathology.

Article Synopsis
  • Interest in metastatic gastric cancer is rising, with the META-GASTRO registry aiming to gather clinical data on oligometastatic patients to guide multimodal treatment, including surgery.
  • The study analyzed data from 383 patients, identifying oligometastatic cases through two definitions: quantitative/anatomical (specific site metastases) and dynamic (percentage based on site), finding relevant statistical significance.
  • The results indicated that 30% of patients fit the quantitative definition and 16% the dynamic definition, with outcomes for conversion therapy similar to those who had surgery after initial chemotherapy.

Article Synopsis
  • The Italian Research Group for Gastric Cancer created a database to assess how a practical approach influences stage IV gastric cancer management and patient outcomes.
  • Data from 383 patients collected between 2018 and September 2022 showed increased use of laparoscopic techniques and a significant difference in surgical treatment strategies compared to past practices.
  • The study found that while different metastatic sites didn't significantly affect survival rates, having multiple sites did worsen survival, indicating that better diagnosis and staging can improve outcomes for patients eligible for curative surgery.

Trastuzumab deruxtecan (T-DXd), a human epidermal growth factor receptor 2 (HER2)-directed antibody-drug conjugate (ADC), has altered the treatment landscape in breast cancer (BC), irrespective of the HR-receptor status. The use of the agent is increasing, despite the finding that exposure to T-DXd increases the risk of interstitial lung disease (ILD), particularly in BC patients. Although T-DXd-related ILD can be potentially severe and life-threatening, most low-grade cases can be treated safely using a multidisciplinary approach comprising early and accurate diagnosis, effective management, close monitoring, and the prompt administration of steroids.

Article Synopsis
  • ALK tyrosine kinase inhibitors (TKIs) are effective against certain tumors but resistance limits their long-term success, with mechanisms of this resistance not well understood in anaplastic large cell lymphoma (ALCL).
  • The study reveals that a survival pathway activated by the tumor microenvironment supports PI3K-γ signaling through CCR7, leading to increased resistance in ALCL cells treated with ALK TKIs.
  • Combining ALK TKI treatment with inhibitors targeting PI3Kγ or CCR7 can reduce resistance and improve outcomes for patients with ALCL, as shown in experiments with cell lines and mouse models.

Despite the high prevalence of upper limb (UL) work-related musculoskeletal disorders (WRMSD) among health care workers (HCWs), little is known about their relationship with exposure to biomechanical risk factors. This study aimed to assess UL activity features under actual working conditions using two wrist-worn accelerometers. Accelerometric data were processed to obtain duration, intensity, and asymmetry of UL use in 32 HCWs during the execution of commonly performed tasks (e.

Article Synopsis
  • Anaplastic Large Cell Lymphoma (ALCL) is a type of non-Hodgkin lymphoma often influenced by a genetic mutation called NPM-ALK, which affects T cell identity and signaling pathways.
  • The study explored how the expression of the immune molecule CD45, crucial for T cell activation, is regulated by the oncogenic NPM-ALK, noting that ALK+ ALCL cells primarily express a specific isoform, CD45RO.
  • Researchers found that inhibition of NPM-ALK increased CD45RO levels and that this regulation required ALK's kinase activity; additionally, knocking out CD45 made ALCL cells more resistant to treatment with ALK inhibitors.

Healthcare professionals generally experience an above-average incidence of low back disorders (LBDs) compared with workers of other professions, and its level of risk is commonly assessed using observational methods such as the MAPO method (Movement and Assistance of Hospital Patients). In this study, we continuously monitored the trunk posture of 30 healthcare workers using a single inertial sensor to: (1) understand whether the MAPO classification is effective in adequately discriminating the risk associated with the time spent in non-neutral trunk postures and (2) characterise the variability of biomechanical exposure among workers employed in wards with the same MAPO index. The results substantially confirm the validity of the MAPO approach in discriminating among wards characterised by different levels of biomechanical exposure associated with the risk of developing LBDs.

Muscular dystrophies (MDs) are heterogeneous diseases, characterized by primary wasting of skeletal muscle, which in severe cases, such as Duchenne Muscular Dystrophy (DMD), leads to wheelchair dependency, respiratory failure, and premature death. Research is ongoing to develop efficacious therapies, particularly for DMD. Most of the efforts, currently focusing on correcting or restoring the primary defect of MDs, are based on gene-addition, exon-skipping, stop codon read-through, and genome-editing.
