Conventional staging paradigm with clinical examination or imaging invariably leads to underestimation of occult metastatic neck disease in oral cavity carcinoma. The advantage of F-FDG PET/CT is in its ability to identify lymph nodes without morphological changes yet harboring occult metastases. We present findings of our study to evaluate diagnostic accuracy of F-FDG PET/CT, in detecting occult cervical lymph node metastasis in carcinoma oral cavity.

Depth of Invasion (DOI) is an important and independent predictor for occult metastasis and recurrence. AJCC staging system (8th edition) has incorporated DOI in the staging of oral cancers. It is an essential parameter in deciding elective neck dissection.

Lymph node staging is a major prognostic factor in colorectal cancer and remains to be the most important criterion for selecting patients for adjuvant therapy. The standard approach for lymph node evaluation is based on manual dissection and histological evaluation of HE-stained slides. For stage III disease (node positive), adjuvant chemotherapy increases the survival rate, while in node-negative stage II disease, in most cases, the chemotherapy is contraindicated due to increased morbidity without real benefit.
