Introduction Squamous cell carcinoma of the oral cavity is one of the top 10 malignancies reported globally. Pakistan has a high incidence of oral cancers due to the prevailing poor lifestyle habits/addictions of Pakistanis, and most patients with squamous cell carcinoma present with stage III or IV locally advanced disease. Recommended guidelines indicate surgery as the mainstay of treatment followed by radiotherapy (RT).

Background: Planning craniospinal irradiation (CSI) with ''field-in-field'' (FIF) homogenization technique in combination with daily, intrafractional modulation of the field junctions is needed to avoid spinal cord overdose. Photon-based techniques for CSI may result in dose inhomogeneity within the treatment volume and usually require a weekly manual shift of the field junctions to minimize the possibility of spinal cord overdose. Nowadays, FIF technique is used to feather out the dose inhomogeneity caused by multiple fields.

Primary mediastinal synovial sarcomas are very rare occurrences with only a few cases reported in literature to date. We present a similar case in an 18-year old male which proved challenging to diagnose and treat. Radiological imaging and tru-cut biopsy results gave rise to suspicion of an unusual malignancy.
