Article Synopsis
  • The study focuses on the anxiety experienced by parents of children undergoing surgery, specifically tonsillectomy, highlighting its negative impact on both parental care and child outcomes.
  • It involved a survey of 147 parents using the Amsterdam Preoperative Anxiety Information Scale (APAIS) to measure anxiety levels, revealing that 40.1% of respondents reported significant anxiety before their child's surgery.
  • Findings indicated certain factors increased anxiety, such as the child's age being under 5, the parent's education level, and whether the parent was the mother, prompting recommendations for targeted support for anxious parents.

Article Synopsis
  • Obese patients present unique challenges for anesthetists due to increased risks of apnea and impaired breathing, making airway protection critical during anesthesia.
  • The study aimed to assess how well the combined Mallampati and Wilson score predicts difficult intubation in obese patients, using the Cormack-Lehane grading system as a standard for difficult intubation.
  • Findings showed that this combined scoring method has a high diagnostic accuracy (94.6%) for identifying patients at risk for difficult intubation, suggesting it’s a reliable and simple tool for anesthetists to use.

Background: To evaluate the incidence and modifiable risk factors of delirium in surgical intensive care unit (SICU) of tertiary care hospital in a low-income and middle-income country.

Methods: We conducted a single cohort observational study in patients over 18 years of age who were admitted to the SICU for >24 hours in Aga Khan University Hospital from January to December 2016. Patients who had pre-existing cognitive dysfunction were excluded.

Patients with Treacher Collins syndrome (TCS) present serious challenges to anesthetist in securing of airway; upper airway obstruction and difficult tracheal intubation are considered complex entity in these patients. This case report describes the significance of transversus abdominis plane (TAP) block as a sole anesthetic choice in appendectomy where airway management can be avoided. A 17-year-old boy, known case of TCS, presented with acute appendicitis underwent emergency laparotomy.

Background And Aims: It is a common practice to add intrathecal lipophilic opioids to local anesthetics to improve the quality of subarachnoid block. This study was designed to find a dose of intrathecal fentanyl, which can improve the quality of surgical anesthesia with minimal side effects in parturients undergoing cesarean section under spinal anesthesia with intrathecal bupivacaine.

Material And Methods: In a prospective randomized double-blind study, 243 parturients undergoing cesarean section under spinal anesthesia were randomly allocated to receive 10, 15, or 25 μg of intrathecal fentanyl with 10 mg of 0.

Background: The objective of study was to determine the pre-anaesthetic status of intracranial pressure (ICP), using ultrasonographic measurement of optic nerve sheath diameter (ONSD) inpatient scheduled for elective tumour craniotomy. The secondary objective was to compare the diagnostic accuracy of ONSD guided rise in ICP with clinical and radiographic parameters. This is prospective observational study, conducted at single neurosurgical theatre of The Aga Khan University over a period of one year.

Article Synopsis
  • - The study investigated the relationship between acute kidney injury (AKI) and its causes in newborns at a specialized hospital in Multan, involving 101 patients from June 2016 to June 2017.
  • - Findings indicated that a majority of the newborns were boys, with common issues like delayed crying, dehydration, and sepsis contributing to AKI, along with a notable neonatal mortality rate of 14.9%.
  • - The research concluded that AKI in newborns leads to significant health complications, with asphyxia and sepsis identified as major causes, and emphasized higher occurrence in boys compared to girls.

Objective: To evaluate the adequacy of serum C-reactive protein (CRP) in diagnosing neonatal sepsis and role of CRP in determining the duration of antibiotic treatment in neonatal sepsis.

Methods: In this validation study, we included 135 neonates with suspected diagnosis of sepsis within duration of nine months from September 2016 to May2017 in Children Hospital Multan. Blood samples were drawn from every neonate for culture sensitivity and measurement of serum C-reactive proteins.

Fundamental medical care includes intravenous (IV) access which provides prompt resuscitation and reliable delivery of analgesics, antibiotics, and vasoactive medication. Difficult access populations, especially in critical area, continue to challenge providers to consider and utilize alternative means to provide IV access. Potential options under such circumstances include intramuscular, intraosseous, and intratracheal drug administration, but in extreme cases where no other options are available, intra-arterial route might be considered.

Article Synopsis
  • The study investigates the incidence of hypotension in patients receiving spinal anesthesia with bupivacaine during cesarean sections, focusing on two different dosing regimens based on height and weight.
  • A randomized double-blind clinical trial involving 60 women revealed that the incidence of hypotension was significantly higher for those whose bupivacaine dose was adjusted only for height compared to those adjusted for both height and weight (56.7% vs. 26.7%).
  • The findings support that calculating the anesthesia dose using both height and weight leads to fewer cases of hypotension and reduced reliance on ephedrine in the management of maternal blood pressure during elective cesarean sections.

Article Synopsis
  • This study aimed to evaluate the effectiveness of paracetamol versus fentanyl for managing pain during dilatation and curettage procedures in female patients.
  • A total of 60 patients were randomly assigned to receive either intravenous paracetamol or fentanyl, with pain levels assessed at multiple intervals post-surgery.
  • Results showed that both groups reported mild pain with no significant difference in pain relief between the two medications, suggesting that paracetamol may be as effective as fentanyl for this procedure.

Article Synopsis
  • The study aimed to assess how accurately the Upper lip bite test and Mallampati test can predict difficult endotracheal intubation in adults undergoing elective surgeries.
  • Conducted at Aga Khan University Hospital, the study involved 324 patients, and data were analyzed using statistical methods to compare the effectiveness of both tests.
  • Results indicated that the Upper lip bite test was more accurate and provided better predictive values than the Mallampati test, suggesting it could be a reliable standalone test for predicting difficult intubation.

Background: Anovulatory infertility due to polycystic ovarian syndrome (PCOS) remains the commonest cause among women of reproductive age group. Although clomiphene citrate (CC) remains the first choice treatment in addition to other medical therapies, laparoscopic ovarian drilling (LOD) is rapidly gaining acceptance as a second line management in patients who failed to respond to CC. This study was done to evaluate the efficacy of LOD in management of PCOS related infertility.
