Post Dural Puncture Subdural Hematoma: A Rare Iatrogenic Complication of Neuraxial Anesthesia.

Although post-dural puncture headache is a well-known complication of neuraxial anesthesia, subdural hematoma following epidural injection is a rare but potentially under-recognized complication. The following is the case of a young female patient with no significant past medical history who developed a persistent and severe headache following the introduction of epidural anesthesia for labor induction. Imaging demonstrated multiple small subdural hematomas with findings concerning intracranial hypotension.

Unusual Case of Renal Tuberculosis in a Patient With Unsuspecting Chronic Back Pain.

Chronic back pain is a common complaint in the United States. In patients from endemic areas, spinal tuberculosis should be a part of the differential diagnosis, especially after the failure of conventional pain management treatments. Although most cases of tuberculosis present with pulmonary complaints, presented here is a case of isolated spinal tuberculosis with contiguous spread to the kidneys with the formation of psoas abscesses.

Fluctuating International Normalized Ratio in Patients Compliant on Warfarin: Could Gastroparesis Be the Cause?

Warfarin is the drug of choice to achieve therapeutic anticoagulation in patients with mechanical heart valves. Factors that interfere with the reliable absorption of warfarin may result in difficult to control international normalized ratio (INR) and can cause significant bleeding complications due to supra-therapeutic INR and thromboembolism from sub-therapeutic INR. The patient's non-compliance is an important factor leading to difficult to control INR but there are additional factors that should be considered in difficult cases when dietary and medication compliance are observed.
