The authors report the case of a dorsal, closed, radiocarpal dislocation with a small posterior marginal fracture of the radius, in an 85-year-old woman, without violent trauma (fall from her height). The dislocation was reduced by traction and the wrist immobilized in a plaster cast for 6 weeks. Six months after the accident, the patient had resumed her activities without pain with a diminished range of motion.

We report a case of spontaneous rupture of extensor pollicis longus of the left hand. This 63 year-old woman was treated with oral corticosteroids for 23 years because of systemic lupus erythematosus. Her hands had no deformity.

The vascular microanastomoses have transformed the bone healing by creeping substitution seen after non-vascularised grafting (a long and often insufficient process) into normal healing of the fracture site. The presence of its own vascular support allows bone healing in such compromised circumstances as sclerosis and infection.

Thanks to the interlocking technic which consists in the fixation of the nail to the bone by means of transfixing screws either in a static or in a dynamic procedure, the method can be extended to fractures localised between both epiphysis of the long bones. After a first period between 1974 and 1983 of development of the method summarized in a first statistic, a second study was performed between 1984 and 1989 in the Center of Traumatology of Strasbourg with 837 cases of fresh fractures of the femur (385) and the tibia (397) treated by locked intra-medullary nailing.
