Background: The versatility of the gastrocnemius muscle for reconstruction of defects in the knee region from the upper third of the calf to the lower third of the thigh is well known. Possible limitations of this flap include difficulties in covering multiple separate wounds in the same area as well as contour deformity because of the flap bulkiness. The findings in this study extend the versatility of the gastrocnemius muscle flap by splitting each head into two segments allowing for the closure of multiple defects.

The single-pedicle transverse rectus abdominis myocutaneous (TRAM) flap is frequently associated with partial flap necrosis. Hyperbaric oxygen has previously been shown to increase the survival of skin flaps, although there has been no investigation of possible beneficial effects of hyperbaric oxygen on survival of the TRAM flap. The present study compares the effectiveness of hyperbaric oxygen therapy, normobaric 100% oxygen, a hyperbaric air-equivalent mixture, and no treatment at all (control group), in the prevention of TRAM flap necrosis in a rat model.

We compared the effect of infiltration with a mixture of 1 per cent lidocaine and two milligrams of morphine in twenty-five patients who were to be managed with a carpal tunnel release with the effect of infiltration with 1 per cent lidocaine only in a second group of twenty-five patients who were to have such a release. In both groups, the injection was administered after inflation of the tourniquet. During the procedure, the patients' movement and vocalization of discomfort did not differ substantially between the groups.

Background: The recognition of a peripheral opioid action has prompted a number of clinical reports demonstrating a prolonged analgesic effect of peripheral opiate. As most studies have used a model of intraarticular instillation of narcotic we examined direct morphine infiltration of the surgical site in a unique clinical model.
Methods: Sixty patients undergoing primary rhinoplasty were entered into this prospective, randomized, double-blind study.
Mastectomy in 16 consecutive patients with carcinoma of the breast was immediately followed by reconstruction with a transverse abdominal, myocutaneous flap. Surgery was performed simultaneously by a team of general surgeons and a team of plastic surgeons. The final cosmetic result was considered very good by most patients.
