AI-Assisted Identification of Primary and Secondary Metabolomic Markers for Postoperative Delirium.

Despite considerable investigative efforts, the molecular mechanisms of postoperative delirium (POD) remain unresolved. The present investigation employs innovative methodologies for identifying potential primary and secondary metabolic markers of POD by analyzing serum metabolomic profiles utilizing the genetic algorithm and artificial neural networks. The primary metabolomic markers constitute a combination of metabolites that optimally distinguish between POD and non-POD groups of patients.

Gene networks for use in metabolomic data analysis of blood plasma from patients with postoperative delirium.

Postoperative delirium (POD) is considered one of the most severe complications, resulting in impaired cognitive function, extended hospitalization, and higher treatment costs. The challenge of early POD diagnosis becomes particularly significant in cardiac surgery cases, as the incidence of this complication exceeds 50 % in certain patient categories. While it is known that neuroinflammation, neurotransmitter imbalances, disruptions in neuroendocrine regulation, and interneuronal connections contribute significantly to the development of POD, the molecular, genetic mechanisms of POD in cardiac surgery patients, along with potential metabolomic diagnostic markers, remain inadequately understood.

PATIENT-CENTERED CARE AND SELF-MANAGEMENT: OPINION OF MILITARY PERSONNEL WITH CORONARY ARTERY DISEASE.

Objective: The aim: Тo determin attitude of military personnel with coronary artery disease to implementation of the principles of patient- centered care and self-assessment of adherence to treatment.

Patients And Methods: Materials and methods: 72 military personnel (male aged 30-58 years) with coronary artery disease, who visited the general practitioners at the Outpatient Care Clinic of the National Military Medical Clinical «Main Military Clinical Hospital» were interviewed anonymously, using the specially designed questionnaire. The patients were divided into two groups: the 1st group with individuals of 49 years old and younger (39 military personnel, mean age 42,8«5,0) and the 2nd group who is 50 years and older (33 military personnel, mean age 53,2«2,4 years).

[Modern ideas on trigeminal ganglion].

The article summarizes data about the structural and functional organization of the sensory ganglion of V cranial nerve - trigeminal, or Gassesrian, ganglion. Information about its discovery, embryonic development, anatomy and topography, features of cyto- and histoarchitectonics are given, the role of neurotransmitters of trigeminal ganglion neurons in nociception mechanisms is reflected, the functional significance of the trigeminal system and clinical aspects of the lesion of the Gasserian ganglion are described.

Glottalized lateral in Rikvani Andi: an acoustic study.

Glottalized sonorants are a rare sound type that has been under scrutiny for a number of reasons of general relevance to the phonetic theory. It has been claimed that the timing of glottalization of glottalized sonorants may shift in accordance with the position in the syllable onset (pre-glottalization) or coda (post-glottalization), to provide a cue for its place of articulation; other studies argued against this claim. The paper investigates acoustic properties of the glottalized lateral in Rikvani Andi, a one-village dialect of Andi (East Caucasian).

Effects of Medicinal Leech-Related Cationic Antimicrobial Peptides on Human Blood Cells and Plasma.

Cationic antimicrobial peptides (CAMPs) are considered as next-generation antibiotics with a lower probability of developing bacterial resistance. In view of potential clinical use, studies on CAMP biocompatibility are important. This work aimed to evaluate the behavior of synthetic short CAMPs (designed using bioinformatic analysis of the medicinal leech genome and microbiome) in direct contact with blood cells and plasma.

Geography and language divergence: The case of Andic languages.

We study the correlation between phylogenetic and geographic distances for the languages of the Andic branch of the East Caucasian (Nakh-Daghestanian) language family. For several alternative phylogenies, we find that geographic distances correlate with linguistic divergence. Notably, qualitative classifications show a better fit with geography than cognacy-based phylogenies.

Type 1 diabetes (T1D) leads to ischemic heart disease and diabetic cardiomyopathy. We tested the hypothesis that T1D differently affects the contractile function of the left and right ventricular free walls (LV, RV) and the interventricular septum (IS) using a rat model of alloxan-induced T1D. Single-myocyte mechanics and cytosolic Ca concentration transients were studied on cardiomyocytes (CM) from LV, RV, and IS in the absence and presence of mechanical load.

Objective: The aim: Тo assess the current approaches to training doctors to manage patients with multimorbidity or comorbidity and the awareness of physicians of the State Institution of Science "Research and Practical Center of Preventive and Clinical Medicine" State Administrative Department (SIS "RPC PCM" SAD) of this problem.

Patients And Methods: Materials and methods: 88 medical doctors of SIS "RPC PCM" SAD were interviewed anonymously, using the specially designed questionnaire. The age of respondents varied from 32 to 72 years; the average age was 53,1±1,2 year; the average length of service was 27,8±1,4 years.

[Applied anatomy of the medial orbital wall].

The article provides an overview of foreign and domestic studies on the anatomy of the medial wall of the orbit. Possible structural variants of the osseus structures of the medial wall and their applied clinical significance are indicated, including those having individual occurrence: additional lacrimal bone, morphological features of the structure of the fossa of the lacrimal sac, location, shape and size of the cells of the ethmoid bone, Onody cells, Haller infraorbital cell, the relative position of the anterior and posterior ethmoid foramina, the presence of additional ethmoid foramina, dysgenesis of the lacrimal bone and the orbital plate of the ethmoid bone. The anatomical prerequisites for the occurrence of intraoperative complications with surgical access to the medial wall of the orbit are described.

Volatile and Intravenous Anesthetics for Brain Protection in Cardiac Surgery: Does the Choice of Anesthesia Matter?

Postoperative neurologic complications have a significant effect on morbidity, mortality, and long-term disability in patients undergoing cardiac surgery. The etiology of brain injury in patients undergoing cardiac surgery is multifactorial and remains unclear. There are several perioperative causative factors for neurologic complications, including microembolization, hypoperfusion, and systemic inflammatory response syndrome.

[The medical ethics of Amatus Lusitanus].

The authors of this study discovered in annals of history the "Oath" of the Portuguese physician Amatus Lusitanus (1511-1568) and translated it into Russian for the first time. The mentioned historical document represents a sort of Code of Honor of European physician of XVI century that both took in ethical norms of Hippocrates and determined one's own rules of doctoring. The ethical principles of Amatus are akin Hippocrates' ones and they are in parallel on seven positions.

Comorbidity in patients with chronic coronary syndromes: prevalence and assessment.

Objective: The aim: To carry out an evaluation of comorbidities in CCS patients having undergone PCI for stable coronary artery diseases (planned) or PCI in myocardial infarction (urgent) with the aim of justifying the best management strategies.

Patients And Methods: Materials and methods: We carried out an analysis of comorbid pathology spreading and the evaluation of multiple comorbidities using of Combined Age Charlson Comorbidity Index (CA-CCI) - in 138 patients below 75 yrs having undergone urgent and planned PCI. These patients were divided into two groups; the Ist one included 60 patients with urgent PCI (angioplasty with stent), the 2nd group - 78 patients with planned PCI.

The immunosuppressive effects of volatile versus intravenous anesthesia combined with epidural analgesia on kidney cancer: a pilot randomized controlled trial.

Background: The aim of this study was to test the hypothesis that the use of inhalational anesthesia leads to higher suppression of the cell-mediated immunity compared to total intravenous anesthesia in patients undergoing kidney cancer surgery under combined low thoracic epidural analgesia and general anesthesia.

Methods: Patients were randomly allocated to either propofol-based (intravenous anesthetic) or sevoflurane-based (volatile anesthetic) anesthesia group with 10 patients in each group, along with epidural analgesia in both groups. Amounts of natural killer (NK) cells, total T lymphocytes, and T lymphocyte subpopulations in the blood samples collected from the patients before surgery, at the end of the surgery and postoperative days 1, 3 and 7 were determined by flow cytometric analysis.

[Use of extracorporeal membrane oxygenation for pulmonary haemorrhage after pulmonary endarterectomy].

Pulmonary haemorrhage is one of the most severe complications of pulmonary endarterectomy. To the most effective methods of combating this potentially fatal complication belongs extracorporeal membrane oxygenation (ECMO). In this article we describe a clinical case report regarding intraoperative use of central veno-arterial ECMO for pulmonary haemorrhage following thromboendarterectomy from the pulmonary arteries.

The historical, formal and logical methods of research, along with system analysis were used in the work.
