Background: Patients with adult T-cell leukemia/lymphoma (ATL) are considered to have worse outcomes after allogeneic hematopoietic stem cell transplantation (allo-HSCT) than patients with other hematological malignancies, owing to high risk of relapse and immunocompromised status. However, no studies have compared transplant outcomes between patients with ATL and those with other hematological malignancies using a large-scale database.

Objectives: To compare transplant outcomes between patients with ATL and those with other leukemias and to identify factors contributing to worse transplant outcomes in ATL patients.

Article Synopsis
  • Allogeneic hematopoietic stem cell transplantation (allo-HSCT) can lead to long-lasting remission in adult T-cell leukemia/lymphoma (ATL) patients, but there's a lack of research on post-transplant outcomes for younger patients (≤49 years).
  • A study found that patients aged <40 years had a significantly better overall survival rate (61.8%) compared to those aged 40-49 years (43.1%), indicating younger patients fare better post-transplant.
  • Moreover, while there were no major differences in survival outcomes between different conditioning regimens, the reduced-intensity conditioning (RIC) regimen resulted in lower non-relapse mortality compared to the myeloablative conditioning (MAC) regimen

Alcohol-associated liver disease (ALD), including alcoholic fatty liver, is a serious problem in many countries, and its economic costs to society are enormous. There is evidence indicating the relations between gut environments and liver disease, and thus, improvement of gut environment is expected to be an effective approach for ALD prevention. In this study, we explored the preventive effect of partially hydrolyzed guar gum (PHGG) on ALD focusing on the gut-liver axis.

Article Synopsis
  • Allogeneic haematopoietic stem cell transplantation (allo-HSCT) is the main treatment for adult T-cell leukemia/lymphoma (ATL) and is influenced by individual human leukocyte antigens (HLAs).
  • A study analyzed data from 829 ATL patients who received transplants, identifying HLA-B62 as linked to lower mortality risk and HLA-B60 with higher mortality risk after transplantation.
  • The findings suggest that specific HLAs can help predict treatment outcomes for ATL patients undergoing allo-HSCT, with potential implications for patient management.

Article Synopsis
  • Epigenomes play a crucial role in correcting abnormal gene expressions in cancer, and targeting histone H3K27me3 is effective for new drug therapies, although the mechanisms are not fully understood.
  • Valemetostat, an EZH1-EZH2 dual inhibitor, has shown promise in clinical trials for aggressive lymphomas by reducing tumor size and providing long-lasting responses even in genetically diverse cancer cases.
  • However, resistance develops over time due to mutations that restore condensed chromatin states, leading to increased H3K27me3 expression and highlighting the need for innovative epigenetic therapies to overcome such challenges in cancer treatment.

In HLA haploidentical stem cell transplantation, patients and donors usually share one HLA haplotype and have one different HLA haplotype (hetero-to-hetero). However, there are rare cases of transplantation from HLA homozygous donors to heterozygous recipients (homo-to-hetero), resulting in mismatches only in the graft-versus-host direction. We previously reported that homo-to-hetero transplants have a lower survival rate in a mouse model than hetero-to-hetero transplants due to stronger graft-versus-host disease (GVHD) but inferior graft-versus-leukemia effect.

Background: We conducted a retrospective study to categorize the cord blood unit (CBU)s to identify the optimal units.

Methods: A total of 8503 adults (female, n = 3592; male, n = 4911) receiving their first single cord blood transplantation (CBT) in 2000-2019 were analyzed. Factors associated with CBUs affecting overall survival (OS) and neutrophil engraftment were selected to create ranked categorization for each outcome, followed by comparison with transplantation using HLA-matched bone marrow (BMT)/peripheral blood stem cell (PBSCT) from unrelated (n = 6052) and related donors (n = 4546).

Chronic graft-versus-host disease (cGVHD) is a multiorgan syndrome with clinical features resembling those of autoimmune diseases. Thus, understanding commonalities in the pathophysiology of cGVHD and autoimmune diseases, such as the presence of disease-risk HLA alleles, is imperative for developing novel therapies against cGVHD. Alloantibodies against H-Y antigens encoded on the Y-chromosome are well-described risk factors for cGVHD in female-to-male transplantation.

Article Synopsis
  • Adult T-cell leukemia/lymphoma (ATLL) is a type of T-cell cancer linked to the HTLV-1 virus, characterized by specific immunophenotypes outlined by the 2017 WHO classification which includes several positive and negative markers.
  • A study assessing immunohistochemical profiles in 117 ATLL cases revealed that 20% did not fit the typical immunophenotype, with significant findings such as high rates of TCR negativity and associations between specific markers and morphological characteristics.
  • No single immunophenotypic marker was found to be predictive of overall survival in ATLL patients, highlighting the complexity of the disease and the need for further research into its diverse profiles.
