South Africa rolled out dolutegravir (DTG) as first-line antiretroviral therapy (ART) in December 2019 to overcome high rates of pretreatment non-nucleoside reverse transcriptase inhibitor drug resistance. In the context of transition to DTG-based ART, this study spatiotemporally analysed detectable HIV viral loads (VLs) prior to- and following DTG rollout in public-sector healthcare facilities in KwaZulu-Natal (KZN) province, the epicentre of the HIV epidemic in South Africa. We retrospectively curated a HIV VL database using de-identified routine VL data obtained from the National Health Laboratory Service for the period January 2018 to June 2022.

Article Synopsis
  • The Healthy Families-PrEP intervention was designed to promote HIV prevention during the period around conception and pregnancy among women in KwaZulu-Natal, South Africa, enrolling HIV-uninfected women in relationships with partners of unknown HIV status.
  • Out of 330 enrolled women, 60% initiated PrEP, with adherence rates varying over time; factors like lower income, alcohol use, and higher HIV stigma negatively impacted adherence.
  • Despite some success in PrEP uptake, there were 11 instances of HIV-seroconversions in participants, none of whom had detectable levels of the drug, highlighting the need for improved strategies to reduce HIV risk in this demographic.

Objective: We aimed to determine whether urine tenofovir (TFV) and dried blood spot (DBS) tenofovir diphosphate (TFV-DP) concentrations are associated with concurrent HIV viraemia.

Design: Cross-sectional study among people with HIV (PWH) receiving tenofovir disoproxil fumarate (TDF)-based antiretroviral therapy (ART).

Methods: We used dual tandem liquid chromatography and mass spectrometry to measure urine TFV and DBS TFV-DP concentrations, and evaluated their associations with concurrent viraemia at least 1000 copies/ml using logistic regression models.

Background: We aimed to compare clinical outcomes after viremia between dolutegravir vs efavirenz-based first-line antiretroviral therapy (ART) as evidence is lacking outside clinical trials in resource-limited settings.

Methods: We conducted a retrospective cohort analysis with routine data from 59 South African clinics. We included people with HIV aged ≥15 years receiving first-line tenofovir disoproxil fumarate, lamivudine, dolutegravir (TLD) or tenofovir disoproxil fumarate, emtricitabine, efavirenz (TEE) and with first viremia (≥50 copies/mL) between June and November 2020.

Article Synopsis
  • Novel point-of-care assays measuring urine tenofovir (TFV) concentrations may enhance adherence monitoring for people living with HIV on antiretroviral therapy (ART), but further research is necessary to assess diagnostic accuracy and its link to viraemia and drug resistance, especially with the global rollout of dolutegravir.
  • In a study involving 124 participants in South Africa, the Abbott point-of-care immunoassay demonstrated high sensitivity (96.1%) and specificity (95.2%) for detecting urine TFV levels compared to a more precise liquid chromatography method.
  • The findings showed a strong association between low urine TFV levels and the presence of viraemia, indicating potential issues with adherence, particularly for those on dolutegravir

Purpose: Elevated levels of inflammation associated with human immunodeficiency virus (HIV) infection are one of the primary causes for the burden of age-related diseases among people with HIV (PWH). Circulating proteins can be used to investigate pathways to inflammation among PWH.

Experimental Design: We profiled 73 inflammation-related protein markers and assessed their associations with chronological age, sex, and CD4 cell count among 87 black South African PWH before antiretroviral therapy (ART).

HIV-1 remains a global health crisis, highlighting the need to identify new targets for therapies. Here, given the disproportionate HIV-1 burden and marked human genome diversity in Africa, we assessed the genetic determinants of control of set-point viral load in 3,879 people of African ancestries living with HIV-1 participating in the international collaboration for the genomics of HIV. We identify a previously undescribed association signal on chromosome 1 where the peak variant associates with an approximately 0.
